【摘要】 目的 探讨血清同型半胱氨酸（Hcy）在妊娠高血压疾病诊断中的价值。方法 选取75例妊娠高血压疾病产妇为疾病组，选择同期208例无相关妊娠并发症产妇为对照组，回顾性分析两组产妇孕中期(14～20⁺⁶周)及晚孕期(34～40⁺⁶周)血清Hcy,绘制受试者工作特征（ROC）曲线分析Hcy的诊断价值。结果 孕中期Hcy两组经Logistic分析OR值为1.127,ROC曲线下面积（AUC）为0.61,截断值为3.97μmol/L时灵敏度为80%,特异度为44%,阳性预测值为34%,阴性预测值为85%;晚孕期OR值为1.357,ROC曲线下面积（AUC）为0.714,截断值为6.89μmol/L时灵敏度为64%,特异度为73%,阳性预测值为46%,阴性预测值为85%。结论 Hcy对妊娠高血压疾病有一定的预测价值，但无法对妊娠高血压疾病严重程度进行有效的鉴别，临床应用时应联合其他指标综合判断。更多还原

【Abstract】 Objective To investigate the diagnostic value of serum homocysteine（Hcy） in hypertensive disorders of pregnancy. Methods 75 pregnant women with hypertensive disorders of pregnancy were selected as the disease group, and a total of 208 women with no related pregnancy complications were randomly selected as the control group, from January 2019 to June 2020. The serum Hcy during second trimester(14～20⁺⁶ weeks) and third trimester(34～40⁺⁶ weeks) in the two groups were retrospectively analyzed, receiver operating characteristic（ROC） curves were drawn to analyze the diagnostic value of Hcy. Results Logistic regression model showed odds ratio（OR） was 1.127,the area under ROC curve（AUC） was 0.61,the cut-off value was 3.97 μmol/L,with sensitivity 80%,specificity 44%,positive predictive value 34%,negative predictive value 85%,with the highest Youden’s index in the second trimester. In the third trimester, the OR value during late pregnancy was 1.357,the area under ROC curve（AUC） was 0.714,a cut-off of 6.89 μmol/L,had sensitivity of 64%,specificity of 73%,positive predictive value of 46% and negative predictive value of 85%. Conclusion The Hcy has certain diagnostic value for hypertensive disorders of pregnancy. But it is not available for distinguishing the severity of hypertensive disorders of pregnancy, and it should be comprehensively judgment combined with other indicators in clinical application.更多还原
