基于镜像神经元理论的动作观察疗法对脑卒中患者吞咽障碍影响的meta分析
Meta-analysis of the impacts of action observation therapy based on mirror neuron theory on dysphagia in stroke patients
【摘要】 目的 系统评价动作观察疗法对脑卒中患者吞咽障碍的影响,为相关研究提供循证依据。方法计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、Web of Science、Cochrane library、Embase中关于动作观察疗法对脑卒中患者吞咽障碍影响的随机对照试验,并追踪参考文献,检索时限为建库至2021年10月。由2名研究者独立筛选文献并交叉核对纳入文献质量,按照Cochrane评价手册5.1.0的质量评价方法评价纳入文献质量。采用RevMan 5.4软件进行meta分析。结果共纳入5篇文献,包括220例脑卒中后吞咽障碍患者。meta分析结果显示,动作观察疗法可以缩短脑卒中后吞咽障碍患者的咽期传输时间[MD=-0.39,95%CI(-0.47,-0.32),Z=10.67,P <0.000 01]、降低肺部感染发生率[OR=0.03,95%CI(0.00,0.32),Z=2.87,P=0.004]、提高经口进食能力[MD=1.24,95%CI(0.88,1.61),Z=6.68,P <0.000 01],尚不能认为动作观察疗法可以缩短脑卒中后吞咽障碍患者口期传递时间[MD=-0.44,95%CI(-0.99,0.11),Z=1.58,P=0.11]。结论 动作观察疗法有利于缩短脑卒中后吞咽障碍患者的咽期传递时间、降低肺部感染发生率、提高患者的经口进食能力,但尚不能认为可以缩短患者的口期传递时间。
【Abstract】 Objective To systematically evaluate the impacts of action observation therapy(AOT) on dysphagia in stroke patients and provide evidence-based reference for relevant researches. Methods The randomized controlled trials about the impacts of AOT on dysphagia in stroke patients in CNKI, Wanfang Database, VIP Database, Chinese Biomedical Literature Database, PubMed, Web of Science, Cochrane library and Embase were retrieved with computer, and the references were tracked. The retrieval time limit was from the establishment of the database to October 2021. The literatures were independently screened by two researchers and the quality of the included literatures was cross-checked. The quality of the included literatures was evaluated according to the quality evaluation method of Cochrane Handbook for Systematic Reviews of Interventions Version 5.1.0. RevMan 5.4 software was used for the meta-analysis. Results A total of 5 literatures were included, and 220 patients with dysphagia after stroke were contained. The meta-analysis results showed that the AOT could shorten the pharyngeal transit time(MD=-0.39, 95%CI[-0.47,-0.32],Z=10.67, P < 0.000 01]), reduce the incidence of pulmonary infection(OR=0.03, 95%CI [0.00, 0.32], Z=2.87,P=0.004), and improve the ability of oral feeding(MD=1.24, 95%CI[0.88, 1.61], Z=6.68, P < 0.000 01) in patients with dysphagia after stroke. However, it couldn’t be considered that AOT could shorten the oral transit time of patients with dysphagia after stroke(MD=-0.44, 95%CI [-0.99, 0.11], Z=1.58, P=0.11). Conclusion AOT is helpful to shorten the pharyngeal transit time, reduce the incidence of pulmonary infection and improve the ability of oral feeding of patients with dysphagia after stroke, but it’s not helpful to shorten the oral transit time of patients.
