老年住院心力衰竭患者γ-谷氨酰转肽酶升高与心功能及院内死亡相关性
Correlation of the Elevated GGT Level with Cardiac Function and In-Hospital Death in Older Inpatients with Heart Failure
【摘要】 目的探讨γ-谷氨酰转肽酶(GGT)升高对老年住院心力衰竭患者院内死亡的预测价值。方法选取2017年8月~2018年8月吉林大学第一医院心血管内科住院治疗的老年心衰患者573例作为研究对象,收集其基线资料、生化指标、超声心动图等信息。根据GGT检查结果将患者分为升高组(>60 U/L, 174例)和正常组(399例)并比较两组之间的差异。应用spearman相关性分析分析GGT与B型利钠肽(BNP)、美国纽约心脏病学会(NYHA)心功能分级、左室射血分数(LVEF)及院内死亡率的相关性,并应用受试者工作特征(ROC)曲线评价其对患者院内死亡的预测价值。结果老年住院心衰患者GGT水平与BNP、NYHA心功能分级、院内死亡呈正相关,与LVEF成负相关。GGT对住院老年心衰患者院内死亡预测的ROC曲线下面积AUC为0.785,约登指数0.562,灵敏度为80.0%,特异度为76.2%。结论老年住院心衰患者GGT水平与其心功能及院内死亡相关。
【Abstract】 Objective To investigate the value of elevated gamma-glutamyl transpeptidase(GGT) level in predicting in-hospital death of older in-patients with heart failure.Methods The older patients with heart failure hospitalized in the First Hospital of Jilin University from August 2017 to August 2018 were enrolled.The general data, biochemical indices, echocardiography and other information were collected.According to serum GGT level, the patients were divided into the elevated group(>60 U/L,174 cases) and the normal group(399 cases).The differences between the two groups were compared.Spearman correlation analysis was used to analyze the correlation of GGT with B-type natriuretic peptide(BNP),New York Heart Association(NYHA) cardiac function grade, left ventricular ejection fraction(LVEF) and in-hospital mortality.ROC curve was used to evaluate the predictive value of GGT level for in-hospital death.Results GGT level in older inpatients with heart failure was positively correlated with BNP level, NYHA grade and in hospital death, and negatively correlated with LVEF.The AUC of GGT for predicting in-hospital death in hospitalized older patients with heart failure was 0.785,Youden index was 0.562,and the sensitivity and specificity were 80.0% and 76.2%,respectively.Conclusion Serum GGT level in older inpatients with heart failure is related to cardiac function and in-hospital death.
