常温下溶血程度对人体血清总IgE的影响

【摘要】 目的 观察常温下溶血程度对人体血清总IgE的影响,为法医学中过敏性猝死鉴定提供参考依据。方法建立人体过敏血样模型,分为全血+IgE组、全血+生理盐水组、血细胞+生理盐水组和实际过敏组,以酶标仪所测得的吸光度值评判溶血程度分级,采用ELISA法检测不同溶血程度下血清总IgE。结果 所有实验组均随着溶血程度的增加,血清总IgE含量呈现先升高后下降的趋势(P <0.05),溶血程度在0~2级时对血清总IgE的检测值的影响较小,实际过敏组溶血程度在0~3级时与血清总IgE浓度存在线性相关(r=0.86),但7 d后IgE浓度低于初始值,不建议作为诊断过敏的依据。结论 司法鉴定中溶血样本可通过建立溶血程度与IgE浓度的回归方程初步推断IgE浓度。

【Abstract】 Objective To observe the effect of hemolysis degree on human serum total immunoglobulin E(IGE) at room temperature, and to provide reference basis for the identification of allergic sudden death in forensic medicine. Methods A human allergic blood sample model was established, which was divided into whole blood IgE group, whole blood normal saline group, erythrocyte normal saline group and allergy experimental group. The degree of hemolysis was evaluated by the absorbance measured by enzyme labeling instrument,and the total Ig E in serum under different degrees of hemolysis was detected by ELISA. Results With the increase of hemolysis degree, the content of serum total IgE increased first and then decreased in all experimental groups. The degree of hemolysis had little effect on the detection value of serum total IgE when it was 0 ~ 2. There was a linear correlation between the degree of hemolysis and the concentration of serum total IgE when it was 0 ~ 3(r = 0.86), but the concentration of IgE was lower than the initial value after 7 days, which could not be used as the basis for the diagnosis of allergy. Conclusion the IgE concentration of hemolytic samples in judicial expertise can be preliminarily inferred by establishing the regression equation between hemolytic degree and IgE concentration.
