【摘要】 目的 观察胆道吻合术治疗胆管扩张症的疗效及安全性。方法 选取2016年8月至2019年2月本院收治的30例胆管扩张症患者作为研究对象，按照随机数字表法分为对照组与研究组，各15例。对照组给予肝管空肠Roux-en-Y吻合术治疗，研究组给予肝管空肠Roux-Y吻合术+矩形瓣术治疗。比较两组临床疗效、手术时间、出血量、住院时间、反流性胆囊炎发生率及并发症发生情况。结果 研究组治疗总有效率明显高于对照组（P<0.05）；研究组手术时间、住院时间明显短于对照组，出血量少于对照组（P<0.05）；研究组反流性胆囊炎发生率明显低于对照组（P<0.05）；研究组近期并发症与远期并发症发生率均明显低于对照组（P<0.05）。结论 肝管空肠Roux-Y吻合术+矩形瓣术治疗胆管扩张症疗效显著，可降低并发症发生率，安全可靠，值得临床推广应用。更多还原

【Abstract】 Objective To observe the efficacy and safety of bile duct anastomosis in the treatment of bile duct dilatation. Methods 30 patients with cholangiectasis treated in our hospital from August 2016 to February 2019 were selected as the research subjects, and they were divided into control group and study group according to random number table method, with 15 cases in each group. The control group was treated with Roux-Y anastomosis, the study group was treated with Roux-Y anastomosis and rectangular valve surgery. The clinical efficacy, operation time, blood loss,hospital stay, incidence of reflux cholecystitis and complications were compared between the two groups. Results The total effective rate of the study group was higher than control group(P<0.05); the operation time and hospital stay in the study group were significantly shorter than those in the control group, the blood loss was less than control group(P<0.05); the incidence of reflux cholecystitis in the study group was significantly lower than that in the control group(P<0.05). The total incidence of recent complications and the total incidence of long-term complications in the study group were higher than control group(P<0.05). Conclusion Roux-Y hepaticojejunostomy + rectangular flap has significant curative effect in the treatment of cholangiectasis, can effectively reduce the incidence of complications, it is safe and reliable, and worthy of clinical application.更多还原
