几丁糖联合HBPA-DDA对肝损伤止血和防粘连的效果评价研究
Evaluation of the Effect of Chitosan Gel Combined with HBPA-DDA on Hemostasis and Anti-adhesion of Liver Injury
【摘要】 为了评价一种超支化聚合物粘合剂(HBPA-DDA)在大鼠体内的肝脏止血效果及安全性,以及与几丁糖凝胶联合使用后的防粘连效果,通过大鼠肝脏出血模型对该材料的即时止血效果、术后生存率和生物安全性进行了探究。结果表明,HBPA-DDA组的平均止血时间比纱布组和康派特医用胶组短。血生化检测指标结果显示,HBPA-DDA组的生物安全性优于纱布组和康派特医用胶组。解剖后观察到HBPA-DDA组较纱布组和康派特医用胶组有较严重的腹腔粘连问题。组织病理学染色结果显示,HBPA-DDA组术后出现一定程度的炎症反应,同时损伤部位出现肉芽组织生长和胶原组织沉积。总体而言,HBPA-DDA的生物相容性较好,与康派特医用胶相比有一定优势。针对HBPA-DDA存在的与腹腔壁粘连严重的情况,进一步通过与医用几丁糖凝胶联合使用来改善。联合使用实验结果显示:腹腔粘连问题得到明显改善,炎症反应有所缓解,促进了伤口愈合,且无其他副作用。HBPA-DDA有优异的止血效果,生物安全性良好,腹腔粘连问题可通过与医用几丁糖凝胶联合使用来改善。
【Abstract】 To evaluate the hemostatic effect and safety of the hyperbranched polymer adhesive( HBPA-DDA) hemostatic hydrogel in rats liver injury and the anti-adhesion effect after combined use with chitosan gel,the instant hemostatic effect,postoperative survival rate,safety and existing problems of the material were explored through the rat liver hemorrhage model. The average hemostasis time of the HBPA-DDA group was shorter than that of the gauze group and the Compont group. The results of blood biochemical test indicators showed that the biosafety of the HBPA-DDA group was also superior to that of the gauze group and the Compont group. After anatomy,it was observed that the HBPA-DDA group had more severe abdominal adhesion problems than the gauze group and the Compont group. The experimental histopathological staining results showed that the HBPA-DDA group had a certain degree of inflammation after operation,at the same time there was granulation tissue growth and collagen tissue production in the damaged part. In general,HBPA-DDA has good biocompatibility and has certain advantages compared with Compont group. The results of the combined use experiment show that the abdominal adhesion problem has been significantly improved,the inflammation has been alleviated,and the wound healing has been promoted without other side effects. HBPA-DDA has an excellent hemostatic effect and a good safety evaluation. The problem of abdominal adhesion can be improved by using it in combination with medical chitosan gel.
