包头地区结直肠癌患者血浆mSEPT9检测分析
Analysis of plasma msept9 in patients with colorectal cancer in Baotou Area
【摘要】 目的:探讨血浆Septin9基因甲基化检测在结直肠癌(Colorectal Cancer, CRC)患者中的意义及临床价值。方法:选取2018年1月至2020年12月在包头市肿瘤医院接受检查的人员为研究对象,共90例;其中经肠镜检查为正常者(无息肉和肿瘤)20例,经肠镜及病理检查结果为息肉者20例,结直肠癌患者50例;分别采静脉血进行mSEPT9检测和Septin9 mRNA表达分析,取粪便进行FOBT试验。结果:CRC患者的Septin9基因甲基化与年龄、性别、民族及癌症的分期均无相关性(P>0.05),与肿瘤的部位和分化程度有关(P<0.05);息肉组和CRC组中,mSEPT9方法比FOBT检出率高(P<0.05),但息肉组和CRC组相比,差异无统计学意义(P>0.05);RT-qPCR检测各组全血标本中Septin 9表达,正常组高于CRC组(4.28±0.036 vs 3.25±0.062 vs 1.01±0.075)(P<0.05),且Septin9 mRNA表达与mSEPT9呈负相关。结论:mSEPT9检测能提高包头地区CRC早期诊断的准确率,可作为CRC早期筛查和诊断的新肿瘤标志物;Septin9基因核苷酸发生甲基化能抑制Septin9 mRNA表达,从而导致抑癌功能丧失,使细胞易发生癌变。
【Abstract】 Objective:To investigate the significance and clinical value of plasma msept9 in the detection of CRC in Baotou area of Inner Mongolia.Methods:From January 2018 to December 2020, 20 normal patients, 20 polyps and 50 CRC were selected from Baotou cancer hospital after colonoscopy. Venous blood samples were taken for mSEPT9 detection and septin9 mRNA expression analysis, and feces samples were taken for FOBT test.Results:In CRC patients, the methylation of septin9 gene was not correlated with age, gender, nationality and cancer stage(P>0.05), but correlated with tumor location and differentiation degree(P<0.05); and in polyp group and CRC group, the detection rate of mSEPT9 method was higher than that of FOBT, and the difference was statistically significant in polyp group(P<0.05), but there was no significant difference in CRC group Significant difference(P>0.05). The expression of septin 9 in normal group was higher than that in polyp group and CRC group(4.28±0.036 vs 3.25±0.062 vs 1.01±0.075, P<0.05). The expression of septin 9 mRNA was negatively correlated with msept9.Conclusion:MSEPT9 can improve the accuracy of early differential diagnosis of CRC, and can be used as a new tumor marker for early screening and diagnosis of CRC. It is also confirmed that the methylation of septin9 gene can inhibit the transcription of normal gene and the loss of tumor suppressive function, which makes the cells prone to carcinogenesis.
