结膜瓣自体血粘附和缝线固定在翼状胬肉手术中的疗效比较
Comparison between Autologous Blood Adhesion and Suture Fixation of Conjunctival Flap in Pterygium Surgery
【摘要】 目的:比较翼状胬肉切除术中自体血粘附和缝线固定两种结膜瓣移植方法的疗效。方法:选择2019年1月~2020年12月在我院手术治疗的原发性翼状胬肉患者共432眼纳入研究,随机分为试验组及对照组。试验组217眼行自体血粘附固定结膜瓣;对照组215眼行缝线固定结膜瓣。术后评估手术时间、伤口愈合时间、结膜瓣位置、伤口愈合不良情况、疼痛症状评分、结膜肉芽肿形成及胬肉复发情况。结果:与对照组相比,试验组的手术时间和术后结膜愈合时间均显著缩短,结膜瓣位置异常发生率增加,术后疼痛症状评分降低,伤口愈合不良和结膜肉芽肿形成的发生率均显著低于对照组(均P<0.05)。两组间胬肉复发率无统计学差异(P>0.05)。结论:翼状胬肉切除术中采用自体血粘附固定结膜瓣手术时间短、并发症少,可作为适宜技术在基层医院推广。
【Abstract】 Objective: To compare the effect of autologous blood adhesion and suture fixation for conjunctival flap transplantation in pterygium excision. Methods: A total of 432 eyes with primary pterygium treated with surgery in our hospital from January 2019 to December 2020 were included, which were randomly divided into the experimental group and the control group. There were 217 eyes in the experimental group underwent pterygium excision and autologous blood adhesion for fixed conjunctival flap. The rest 215 eyes in the control group were subjected with pterygium excision and suture fixation for conjunctival flap. The follow-up indexes were evaluated, including operation time, postoperative wound healing time, abnormal position of conjunctival flap, poor conjunctival healing, postoperative pain score, conjunctival granuloma and recurrence of pterygium. Results: Compared to the control group, the operation time and conjunctival healing were both significantly shortened, the incidence of abnormal conjunctival flap position was increased, the postoperative pain score was decreased, and the incidence of poor wound healing and conjunctival granuloma formation were significantly lowered in the experimental group(all P<0.05). There was no significant difference in the recurrence rate of pterygium between the two groups(P>0.05). Conclusion: The conjunctival flap fixed by autologous blood adhesion in pterygium excision has the advantages of shorter operation time and less complications, which can be popularized as an appropriate technology in grass-roots hospitals.
