全自动生化分析仪检测中黄疸指数的临床应用
The Clinical Application of the Icteric Index Detected by the Automatic Biochemical Analyzer
【摘要】 目的 分析临床实验室标本黄疸(黄疸指数≥2mg/dL)发生率及相关影响因素,探讨黄疸指数(icteric index, II)的临床应用价值。方法 回顾性选取北京中医药大学东方医院2021年1月至12月28 825例生化血清标本。采用Pearson相关性分析、线性回归分析验证黄疸指数和血清总胆红素(total bilirubin, TBIL)测定值的相关性。采用χ~2检验、t检验分析不同患者就诊类型、性别、年龄、临床科室标本黄疸率的变化。结果 2021年总标本黄疸率为1.84%;黄疸指数和血清胆红素浓度高度相关。住院部标本黄疸率明显高于其他部门(P<0.05);男性标本黄疸率显著高于女性(P<0.05);儿童组标本黄疸率最高(P<0.05);肿瘤科标本黄疸率明显高于其他科室(P<0.05)。结论 标本黄疸率与患者就诊类型、性别、年龄、临床科室有关。黄疸指数有利于准确评估分析前标本质量,提高实验室检验水平,优化临床诊疗和患者护理。
【Abstract】 Objective To analyze the incidence of jaundice(icteric index≥2mg/dL) in clinical laboratory specimens and related influencing factors, and to explore the clinical application value of the icteric index-(II).Methods 28 825 biochemical serum specimens from January to December, 2021 were selected retrospectively from Dongfang Hospital of Beijing University of Chinese Medicine. Pearson correlation analysis and linear regression model were used to verify the correlation between the icteric index and serum total bilirubin(TBIL) measurements.χ~2 test and t-test were conducted to analyze changes in the icteric rate of specimens from different patient visit types, gender, age, and clinical departments.Results The total specimen icteric rate of 2021 was 1.84%; and the icteric index was highly correlated with the serum bilirubin concentration. The icteric rate of specimens from the Inpatient Department was significantly higher than that of other departments(P<0.05). The icteric rate of male specimens was significantly higher than that of females(P<0.05). The icteric rate of specimens from the pediatric group was the highest(P<0.05). The icteric rate of specimens from the Oncology Department was significantly higher than that of other departments(P<0.05).Conclusion The icteric rate of specimens is related to the type of patient visits, gender, age, and clinical departments. The icteric index is helpful for accurately assessing the quality of pre-analytical specimens, improving laboratory performance, and optimizing clinical practice and patient care.
