借鉴流派传承经验 助力临证思维训练

【摘要】 为寻求符合中医成才规律的教学方法,借鉴传承有序、活力充沛的中医学术流派的临证思维及决策经验,培训住院医师是一种有益的尝试。中医学术流派是中医学在历史发展中形成的具有独特学术思想或技艺的临床诊疗体系。流派传承人的临证思维的系统化积累形成了流派的学术思想。流派生命的延续源于一代代的学术思想传承,活力则源于历代传人通过临证思维产生的疗效。我们借助长安米氏内科流派资源,将流派人本文化与核心价值观结合,培育住院医师优良医德以坚守生命至上的临证思维底线;通过领读流派学术著作传承学术思想,用流派历代传人临证思维及决策特征启发住院医师建立临证思维模式;编制规范化、标准化的流派诊疗方案,通过临床带教传授实用诊疗技艺,培养住院医师建立有效、高效的临证思维。

【Abstract】 In order to seek a teaching method that conforms to the rule of Traditional Chinese Medicine(TCM) talent training, it is a useful attempt to train residents on the clinical thinking and decision-making experience of the traditional orderly and energetic TCM academic schools. The academic schools of TCM are various clinical diagnosis and treatment systems with unique academic thoughts or skills formed in the historical development of TCM. The systematic accumulation of the critical thinking of the inheritors of different schools forms the academic thoughts of each school.The continuation of each school’s life comes from the inheritance of academic thoughts from generation to generation, and its vitality comes from the curative effect of clinical thinking produced by successive generations. With the help of the resources of Chang’an Mi School of Internal Medicine, we combined the humanistic culture of the school with the core values, and cultivated the excellent medical ethics of residents to stick to the bottom line of clinical thinking that life came first. Through reading the academic works of the school, the academic thoughts were passed on and the characteristics of clinical thinking and decision-making of the descendants of the school were used to inspire the residents to establish clinical thinking mode. We also prepared standardized school diagnosis and treatment scheme, taught practical diagnosis and treatment skills through clinical teaching, and trained residents to establish effective and efficient clinical thinking.
