冠心病病人IL-35、IL-6、Cys C水平变化及其与冠状动脉病变支数、冠状动脉成像钙化积分的相关性

【摘要】 目的观察冠心病病人血清白介素-35(IL-35)、白介素-6(IL-6)、胱抑素C(Cys C)水平变化,并分析其与冠状动脉病变支数及冠状动脉成像(CTA)钙化积分(CACS)的相关性。方法选取2017年5月—2019年4月四川绵阳四〇四医院心血管内科拟诊为冠心病的病人95例为研究对象,依据冠状动脉造影(CAG)结果将其分为冠心病组(55例)、非冠心病组(40例),比较两组血清IL-35、IL-6、Cys C表达水平、CACS积分,比较不同冠状动脉病变、不同钙化程度冠心病病人血清IL-35、IL-6、Cys C水平,分析冠心病病人血清IL-35、IL-6、Cys C水平、冠状动脉病变支数、CACS积分之间的相关性。结果冠心病组血清IL-35低于非冠心病组(P<0.01),IL-6、Cys C水平高于非冠心病组(P<0.01);冠心病组右冠状动脉、左前降支、左回旋支、左主干CACS积分与CACS总积分均高于非冠心病组(P<0.01);冠心病病人中,随着冠状动脉病变支数的增加;IL-35逐渐降低,Cys C和IL-6逐渐升高,差异均有统计学意义(P<0.05或P<0.01);双支病变病人IL-35低于单支病变病人,Cys C和IL-6高于单支病变病人,差异均有统计学意义(P<0.05或P<0.01)。随着冠心病病人冠状动脉钙化程度增加,IL-35含量减少(P<0.01),IL-6含量升高(P<0.01),Cys C水平也随冠状动脉钙化程度增加有增长趋势,但差异无统计学意义(P>0.05)。相关性分析显示,冠心病病人血清IL-35与CACS积分、冠状动脉病变支数呈负相关(P<0.05),而IL-6、Cys C水平与CACS积分、冠状动脉病变支数呈正相关(P<0.05),IL-6与Cys C也呈正相关(P<0.05)。结论冠心病病人血清IL-35低表达,而IL-6、Cys C高表达,且均与CACS积分、冠状动脉病变支数有一定相关性,可能参与冠心病发生发展。

【Abstract】 Objective To analyze the relationship between serum interleukin-35(IL-35),interleukin-6(IL-6),cystatin-C(Cys C) levels and the number of coronary artery lesions, coronary artery calcification score(CACS) in patients with coronary heart disease(CHD).Methods Ninety-five suspect CHD patients were enrolled, and divided into CHD group(n=55) and non-CHD group(n=40) according to coronary angiography(CAG).The expression levels of serum IL-35,IL-6,Cys C,and CACS were compared between two groups.The levels of serum IL-35,IL-6,and Cys C in CHD patients with different coronary artery lesions and different calcification degrees were analyzed.Correlation between IL-35,IL-6,Cys C levels and the number of coronary artery lesions and CACS in patients with CHD were studied.Results The serum IL-35 level in CHD group was lower than that in non-CHD group(P<0.01),while the levels of IL-6 and Cys C were higher than those in non-CHD group(P<0.01).The right coronary artery, left anterior descending, left circumflex artery, left main coronary artery and total CACS in CHD group were higher than those in non-CHD group(P<0.01).Among CHD patients, serum IL-35 in patients with multi-vessel lesions was lower than that with single-vessel lesions and double-vessel lesions, while IL-6 and Cys C levels were higher than those with single-vessel lesions and double-vessel lesions(P<0.05 or P<0.01).Serum IL-35 in patients with double-vessel lesions was lower than that with single-vessel lesions, while IL-6 and Cys C levels were higher than those with single-vessel lesions(P<0.05 or P<0.01).With increase of coronary calcification, IL-35 decreased and IL-6 increased in CHD patients(P<0.01),and Cys C level showed increasing trend(P>0.05).Correlation analysis showed that serum IL-35 was negatively correlated with CACS and the number of coronary lesions in CHD patients(P<0.05),while IL-6 and Cys C levels were positively correlated with CACS and the number of coronary lesions(P<0.05).IL-6 was also positively correlated with Cys C(P<0.05).Conclusion Serum IL-35 lowly expressed in CHD patients, while IL-6 and Cys C highly expressed, and related to CACS and the number of coronary artery lesions, which may be involved in occurrence and development of CHD.
