小儿先天性梨状窝瘘影像学表现

【摘要】 目的观察小儿先天性梨状窝瘘(CPSF)影像学表现。方法 22例CPSF患儿(2例新生儿),接受平扫及增强CT(n=20)、平扫及增强MR(n=2)及X线造影(n=3),其中2例先后接受CT及MR检查,1例先后接受CT及X线造影。结果 22例中,CPSF位于左侧21例、右侧1例。新生儿CPSF以颈部含气囊性包块伴气管受压、变形、狭窄为特征性CT、MRI表现。儿童CPSF的主要CT和MRI表现为梨状窝变小,甲状腺变形、肿大、密度减低,甲状腺上极气泡征等;X线造影可见经梨状窝延伸的瘘道或瘘管影。结论小儿CPSF影像学表现具有一定特征性,有助于诊断。

【Abstract】 Objective To observe imaging manifestations of congenital pyriform sinus fistulas(CPSF) in children. Methods Totally 22 children(including 2 newborns) with CPSF underwent plain and enhanced CT(n=20), plain and enhanced MR scanning(n=2) and X-ray fistulography(n=3). Among them 2 children underwent CT and MR, 1 child underwent CT and X-ray fistulography. Results CPSF located on the left side in 21 cases, on the right in 1 case. The main CT and MRI findings of neonatal CPSF included simple cystic lesion containing air pocket, with tracheal compression and lumen deformation, of CPSF in children mainly included smaller pyriform fossa, sinus tract or fistula connected with piriform fossa, enlarged volume, low density and morphologic change of thyroid, bubble sign in the inner side of upper pole of thyroid, etc.The characteristic X-ray fistulography feature of CPSF in children was sinus tract or fistula extended through the piriform fossa. Conclusion Imaging manifestations of CPSF in neonates and children had characteristics being helpful to diagnosis of this disease.
