间断利尿预防慢性心力衰竭利尿剂抵抗的机制研究

【摘要】 目的:探究间断利尿预防慢性心力衰竭利尿剂抵抗的机制。方法:选取20只8~10周龄通过缩窄升主动脉建立的大鼠心力衰竭模型为研究对象,将其随机分为持续利尿剂组与间断利尿剂组,每组10只。比较两组实验起点及第6、12、16、20周血液指标(PRA、Ang-Ⅱ、ALD及NT-proBNP)、心脏超声检查结果(HR、LVEDD、LVESD、EF、LVFS、LVAWD/S及LVPWD/S)、肾远曲小管增生情况及心肌纤维化情况。结果:两组实验起点血液指标、心脏超声检查结果比较,差异均无统计学意义(P>0.05)。间断利尿剂组第6、12、16、20周血液指标、心脏超声检查结果均显著优于持续利尿剂组,肾远曲小管PI及心肌CVF均显著低于持续利尿剂组,差异均有统计学意义(P<0.05)。结论:间断利尿可有效预防慢性心力衰竭利尿剂抵抗的情况,对心肾等方面不良影响的防控作用较好。

【Abstract】 Objective:To investigate the mechanism of intermittent diuresis in preventing diuretic resistance of chronic heart failure.Method:A total of 20 rats of 8 to 10 weeks old with heart failure by constricting ascending aorta were as the study objects,and they were randomly divided into continuous diuresis group and intermittent diuresis group,10 rats in each group.Then the blood indexes (PRA,Ang-Ⅱ,ALD and NT-proBNP) and echocardiography results (HR,LVEDD,LVESD,EF,LVFS,LVAWD/S and LVPWD/S) at experimental starting point,the 6th,12th,16th and 20th week,renal distal convoluted tubule hyperplasia situation and myocardial fibrosis situation of two groups were compared.Result:The blood indexes and echocardiography results of two groups at experimental starting point were compared,there were no statistically significant differences (P>0.05).The blood indexes and echocardiography results of intermittent diuresis group at the 6th,12th,16th and 20th week were significantly better than those of continuous diuresis group,the distal convoluted tubule PI and myocardial CVF were significantly lower than those of continuous diuresis group,there were statistically significant differences (P<0.05).Conclusion:The intermittent diuresis can effectively prevent the diuretic resistance of chronic heart failure,and its control effect for the adverse effects on heart and kidney is better.
