【摘要】 目的:探讨加长柄水泥型关节置换治疗高龄粗隆间骨折患者的临床效果及对Harris评分的影响。方法:选取2017年6月-2019年5月入住本院的高龄粗隆间骨折患者72例。将患者随机分为对照组和研究组,每组36例。对照组应用微创近端髓内钉治疗,研究组应用加长柄水泥型关节置换治疗。比较两组的临床疗效、手术相关指标及并发症发生情况。结果:研究组疼痛、关节活动、关节畸形、日常活动能力和步态评分及总评分均优于对照组,差异均有统计学意义(P<0.05)。研究组下床时间、手术时间、住院时间均短于对照组,术中出血量少于对照组(P<0.05)。研究组的疗效优于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:对高龄粗隆间骨折患者应用加长柄水泥型关节置换术,并发症相对较少,Harris评分较高,术中出血量较少,手术时间及住院时间较短,疗效佳,可以推广应用。更多还原

【Abstract】 Objective:To investigate the clinical effect of cement joint replacement with extended handle in the treatment of elderly patients with intertrochanteric fractures and its influence on Harris score.Method:A total of 72 elderly patients with intertrochanteric fracture admitted to our hospital from June 2017 to May 2019 were selected.The patients were divided into the control group and study group,36 cases in each group.The control group was treated with minimally invasive proximal intramedullary nailing,and the study group was treated with cement joint replacement with extended handle.Clinical efficacy,operation-related indexes and complications were compared between the two groups.Result:The pain,joint motion,joint deformity,daily activity ability,gait scores and total score of the study group were better than those of the control group,the differences were statistically significant (P<0.05).The time of getting out of bed,operation time and hospitalization time of the study group were shorter than those of the control group,and the amount of intraoperative blood loss was less than that of the control group (P<0.05).The curative effect of the study group was better than that of the control group,the difference was statistically significant (P<0.05).The complication rate in the study group was lower than that in the control group,the difference was statistically significant (P<0.05).Conclusion:The application of cement joint replacement with extended handle in elderly patients with intertrochteric fracture has relatively fewer complications,higher Harris score,less intraoperative blood loss,shorter operation time and hospitalization time,good efficacy and can be popularized.更多还原
