【摘要】 目的观察骨化三醇联合摄精消白胶囊对早期糖尿病肾病(diabetic nephropathy,DN)患者尿蛋白及骨密度的影响。方法选取2018年7月—2019年12月就诊于本院的早期DN患者94例,按照随机数字表法随机分为3组。对照组29例予以降糖、降压治疗;西药组33例在对照组治疗基础上,予以骨化三醇口服治疗;联合组32例在对照组治疗基础上,予以骨化三醇联合摄精消白胶囊口服治疗。连续治疗3个月后,比较3组患者治疗前后肾功能、尿蛋白、血钙、血磷、甲状旁腺激素(parathyroid hormone,PTH)及骨密度水平。结果治疗后,西药组、联合组血磷、血清肌酐、PTH水平及24h尿蛋白、尿白蛋白排泄率显著低于对照组(P<0.05)。西药组、联合组血钙、血清25(OH)D3及腰椎、股骨颈骨密度水平显著高于对照组(P<0.05)。联合组肌酐水平、尿白蛋白排泄率显著低于西药组(P<0.05)。结论骨化三醇及摄精消白胶囊均可有效降低早期DN患者尿蛋白及肌酐水平、改善钙磷代谢、提高骨密度,两者联合使用,疗效更为显著。更多还原

【Abstract】 Objective To observe the effects of calcitriol combined with Shejing Xiaobai capsule on urinary protein and bone mineral density in patients with early diabetic nephropathy(DN).Methods From July 2018 to December 2019,ninety-four early DN patients were randomly divided into 3 groups.In the control group,29 cases were treated with hypoglycemic and hypotensive therapy.On the basis of the control group,33 cases of western medicine group were treated with calcitriol.On the basis of control group,32 cases of the combined group were treated with calcitriol combined with Shejing Xiaobai capsule orally.After 3 months of continuous treatment,the renal function,the urine protein,the blood calcium,the blood phosphorus,the parathyroid hormone(PTH)and the bone mineral density levels were compared between the three groups.Results After treatment,the serum levels of phosphorus,creatinine,PTH,24 hurinary protein and urinary albumin excretion rate in western medicine group and combined group were significantly lower than those in control group(P<0.05).The serum levels of calcium,25(OH)D3 and bone density of lumbar and femoral neck in western medicine group and combined group were significantly higher than those in control group(P<0.05).The serum levels of creatinine and urinary albumin excretion rate in the combined group were significantly lower than those in the western medicine group(P<0.05).Conclusion Both calcitriol and Shejing Xiaobai capsule could effectively reduce the level of urine protein and creatinine,improve calcium and phosphorus metabolism,increase bone mineral density in early DN patients,and the combination of the two is more effective..更多还原
