高频振荡通气辅以西地那非治疗新生儿呼吸衰竭伴持续性肺动脉高压的疗效探讨

【摘要】 目的分析高频振荡通气辅以西地那非治疗新生儿呼吸衰竭伴持续性肺动脉高压的临床疗效。方法 50例新生儿呼吸衰竭伴持续性肺动脉高压患儿,采用随机数字表法分为对照组和观察组,每组25例。对照组患儿给予高频振荡通气治疗,观察组患儿给予高频振荡通气辅以西地那非治疗。比较两组治疗效果、生命体征恢复平稳时间、肺动脉压恢复正常时间及治疗前后二氧化碳分压、动脉血氧分压、肺动脉收缩压、血氧饱和度。结果观察组治疗总有效率为96.00%,高于对照组的76.00%,差异有统计学意义(P<0.05)。观察组生命体征恢复平稳时间(1.24±0.57)d、肺动脉压恢复正常时间(2.67±0.24)d均短于对照组的(3.56±1.24)、(4.56±1.14),差异有统计学意义(P<0.05)。治疗后,两组二氧化碳分压、动脉血氧分压、肺动脉收缩压及血氧饱和度均优于治疗前,且观察组均优于对照组,差异有统计学意义(P<0.05)。结论高频振荡通气辅以西地那非治疗新生儿呼吸衰竭伴持续性肺动脉高压的疗效确切,可有效缩短治疗时间,改善预后。

【Abstract】 Objective To analyze the clinical efficacy of high-frequency oscillatory ventilation combined with sildenafil in the treatment of neonatal respiratory failure with persistent pulmonary hypertension. Methods A total of 50 neonates with respiratory failure and persistent pulmonary hypertension were divided into control group and observation group according to random numerical table, with 25 cases in each group. The control group was treated with high-frequency oscillatory ventilation, and the observation group was treated with high-frequency oscillatory ventilation and sildenafil. The therapeutic effect, the time for the vital signs to recover stably, the time for the pulmonary artery pressure to return to normal, and the partial pressure of carbon dioxide, arterial oxygen, pulmonary artery systolic pressure and blood oxygen saturation before and after treatment were compared between the two groups. Results The total effective rate of the observation group was 96.00%, which was higher than 76.00% of the control group, and the difference was statistically significant(P<0.05). The time for the vital signs to recover stably and the time for the pulmonary artery pressure to return to normal of the observation group were(1.24±0.57) and(2.67±0.24) d, which were shorter than(3.56±1.24) and(4.56±1.14) d of the control group, and the difference was statistically significant(P<0.05). After treatment, the partial pressure of carbon dioxide, partial pressure of arterial oxygen, pulmonary artery systolic blood pressure and blood oxygen saturation of the two groups were better than those before treatment, and the observation group was better than the control group. All the difference was statistically significant(P<0.05). Conclusion Combination of high-frequency oscillatory ventilation and sildenafil has affirmative efficacy in the treatment of neonatal respiratory failure with persistent pulmonary hypertension, which can shorten the treatment time and improve the prognosis of patients.
