环介导等温扩增技术检测呼吸道病毒方法的建立及应用
Establishment and application of Loop-mediated isothermal amplification method for detecting respiratory virus
【摘要】 目的探讨在呼吸道病毒检测中建立环介导等温扩增技术(LAMP)的可行性,并评价其临床应用价值。方法建立乙型流感病毒、腺病毒、呼吸道合胞病毒的LAMP检测方法,应用本方法和直接免疫荧光法(DFA)平行测定74份鼻咽拭子标本的3种呼吸道病毒的感染标志物。并以DFA为参考方法,比较两种检测技术在检出率、灵敏度、特异度等方面的差异,以及在预测结果上是否具有一致性。结果 LAMP与DFA检测乙型流感病毒检出率分别为27.03%和18.92%,腺病毒16.22%和22.97%,呼吸道合胞病毒均为6.76%,McNemar检验显示两者结果差异无统计学意义(P>0.05);检测3种病毒时相对参考方法(DFA)的灵敏度:92.86%、52.95%、80.00%,特异度:88.33%、85.14%、98.55%,整体符合率:89.19%、85.14%、97.30%,经Kappa检验显示两者在预测结果上一致性一般或较好(Kappa值分别为0.697、0.532、0.786)。结论 LAMP与参考方法(DFA)有较高的一致性,且LAMP相对DFA具有实验条件要求较低、操作简单、检测周转时间(TAT)短等优点。
【Abstract】 Objective To explore the feasibility of establishing loop-mediated isothermal amplification(LAMP)technology in the detection of respiratory viruses,and evaluate its clinical application value.Methods Establish LAMP detection methods for influenza B virus,adenovirus,and respiratory syncytial virus,This method and Direct fluorescence assay(DFA)were used to determine the infection markers of 3 respiratory viruses in 74 nasopharyngeal swabs in parallel.DFA was used as a reference method to compare the difference of detection rate,sensitivity,specificity and other aspects between the two detection techniques,and whether there was consistency in the prediction of diagnostic results.Results The detection rates of influenza B virus by LAMP and DFA were 27.03% and 18.92%,adenovirus 16.22%and 22.97%,and respiratory syncytial virus were both 6.76%.The McNemar test showed that the difference between the two results was not statistically significant(P>0.05);The sensitivity of the LAMP method relative to the reference method(DFA)when detecting 3 viruses:92.86%,52.95%,80.00%,the specificity:88.33%,85.14%,98.55%,and the overall coincidence rate:89.19%,85.14%,97.30%;Kappa test showed that the consistency of the two methods was average or good(kappa values were 0.697,0.532 and 0.786,respectively).Conclusion LAMP has a higher consistency with the reference method DFA,and compared with DFA,LAMP has the advantages of lower experimental conditions,simpler operation,and shorter turnaround time(TAT).
