【摘要】 目的评价聚乙二醇脂质体多柔比星(PLD)在抗肿瘤中的临床应用现状,为临床合理用药提供依据。方法对本院2020年1月—12月期间住院使用过PLD的1328例恶性肿瘤患者的用药情况进行回顾性分析,参考药品说明书及临床实践指南等的推荐,对患者治疗情况进行统计和评价。结果共有10种适应证使用PLD,均为超说明书用药,其中复发或转移性乳腺癌、复发卵巢癌、霍奇金淋巴瘤、非霍奇金淋巴瘤及多发性骨髓瘤的超说明书应用有一定的循证医学依据,属于合理用药;而子宫肉瘤、子宫内膜癌、子宫颈癌、骨恶性肿瘤及软组织肉瘤的超说明书用药循证医学证据不足,不推荐使用。结论 PLD超说明书用药普遍存在,应加强PLD应用的管理,特别是超说明书用药的监管规范,确保药物安全合理使用。更多还原

【Abstract】 Objective To evaluate the clinical application of pegylated liposomal doxorubicin(PLD)for cancer patients and provide reference for rational drug use. Methods We retrospectively analyzed 1328 patients who were given PLD in our hospital from January to December in 2020. The basic information of patients was collected, and the treatment of the patients was evaluated with drug instructions and clinical practice guidelines. Results Ten indications for off-label use of PLD were found. The off-label use of PLD for recurrent or metastatic breast cancer, recurrent ovarian cancer,Hodgkin’s lymphoma, non-Hodgkin’s lymphoma and multiple myeloma was more or less supported by evidence-based medicine and might be considered rational. While evidence for the off-label use of uterine sarcoma, endometrial cancer, cervical cancer, bone malignancy and soft tissue sarcoma was insufficient, and was regarded as irrational. Conclusion The off-label use of PLD is common in our hospital. It is important to monitor and ensure the safe and rational use of PLD.更多还原
