【摘要】 [目的]比较两种入路半髋置换治疗老年痴呆股骨颈骨折的早期疗效。[方法] 2018年7月—2020年5月80例80岁以上老年痴呆股骨颈骨折患者随机分为两组。其中,40例采用直接前方入路(direct anterior approach, DAA),40例采用后外侧入路(posterolateral approach, PLA)行半髋置换术。比较两组早期临床结果。[结果]DAA组手术失血量、切口长度、术后下床时间、完全负重活动时间均显著优于PLA组(P<0.05),DAA组并发症发生率显著低于PLA组(7.50%vs 35.89%,P<0.05)。[结论] DAA入路半髋置换治疗老年痴呆股骨颈骨折的早期临床结果优于PLA入路。更多还原

【Abstract】 [Objective] To compare the early clinical outcomes of two approaches for hip hemiarthroplasty for femoral neck fractures in the elderly with dementia. [Methods] From July 2018 to May 2020, a total of 80 patients who were over 80 years old, and suffered from senile dementia and femoral neck fractures were randomly divided into two groups. Among them, 40 patients were by using the direct anterior approach(DAA), while the other 40 patients were through posterolateral approach(PLA) for hip hemiarthroplasty. The early clinical results were compared between the two groups. [Results] The DAA group was significantly superior to the PLA group in terms of intraoperative blood loss, incision length, time to get out of bed and time to resume full weight-bearing activity after operation(P<0.05).The DAA group had complication rate significantly lower than the PLA group(7.50% vs 35.89%, P<0.05). [Conclusion] The DAA is considerably better than the PLA for hip hemiarthroplasty for femoral neck fractures in the elderly with dementia in term of early clinical outcomes.更多还原
