EGFR在不同分子分型乳腺癌中的表达及其与临床病理指标的关系

【摘要】 目的探讨EGFR在不同分子分型乳腺癌中的表达;分析EGFR与临床病理指标的关系。方法用免疫组化的方法 ,检测EGFR在325例乳腺癌肿瘤组织中的表达,回顾性分析其与分子分型、月经状态、病理类型、病理分期、组织学分级的关系。结果 EGFR在腔面A型、腔面B型、HER-2过表达型和三阴型乳腺癌中的阳性表达率分别为:24.18%、16.44%、51.43%和75.47%,除腔面A型和B型的差异无统计学意义外,其余各型之间的差异均有统计学意义(P<0.05)。EGFR的阳性表达与年龄无关。相较与浸润性癌,导管原位癌阳性表达率高;相较于特殊类型乳腺癌,浸润性导管癌阳性表达率高,但差异均无统计学意义。Ⅲ期的浸润性乳腺癌,EGFR的阳性表达率最高,与Ⅰ期、Ⅱ期相比,差异有统计学意义(P<0.05)。Ⅲ级的浸润性乳腺癌,EGFR的阳性表达率最高,与Ⅰ级、Ⅱ级相比,差异有统计学意义(P<0.05)。结论EGFR在三阴型乳腺癌中的阳性表达率最高,在浸润性导管癌中,EGFR的阳性表达与病理分期、组织分级密切相关。

【Abstract】 Objective To analyze the expression of EGFR in different molecular types of breast cancer. To analyze the correlation between EGFR and clinicopathological indexes. Methods Detected the expression of EGFR in 325 cases of breast cancer was detected by immunohistochemistry, and the correlation between the molecular typing, menstrual state, pathological stage, histological grade and pathological type was analyzed. Results The positive expression rates of EGFR were 24.18%, 16.44%, 51.43% and 75.47% in Luminal A, Luminal B, HER-2 overexpression and three negative breast cancer. Except for the difference between Luminal A and Luminal B, the differences among the other types were statistically significant(P <0.05). The positive expression of EGFR was not related to age. Compared with invasive carcinoma,the positive expression rate of ductal carcinoma in situ was higher,compared with the special type of breast cancer, the positive expression rate of invasive ductal cancer was higher, but the difference was not statistically significant. In stage Ⅲ invasive breast cancer, the positive expression rate of EGFR was the highest, compared with stage Ⅰ and stage Ⅱ, the difference was statistically significant(P <0.05). In grade Ⅲ invasive breast cancer, the positive expression rate of EGFR was the highest, compared with grade Ⅰ and grade Ⅱ, the difference was statistically significant(P <0.05). Conclusion The positive expression rate of EGFR is the highest in three negative breast cancer. In invasive ductal cancer, the positive expression of EGFR is closely related to pathological stage and tissue grade.
