无框架立体定向血肿清除术与开颅血肿清除术治疗小脑出血的Meta分析

【摘要】 目的分析无框架立体定向血肿清除术与开颅血肿清除术治疗小脑出血的疗效。方法按照设定的检索策略,广泛检索各大数据库,利用RevMan5.3软件对纳入的研究按照不同观察指标(手术时间、住院时间、术中出血量、术后日常活动能力、术后并发症)进行Meta分析。结果收集符合标准的10个临床研究,共591例病例,其中定向组297例,开颅组294例。2组患者在术后日常活动能力及术后再出血方面差异无统计学意义(P>0.05),定向组在手术和住院时间、术中出血量、术后并发症更有优势,差异具有统计学意义(P<0.05)。结论无框架立体定向血肿清除术作为一种微创手术方式,在治疗小脑出血方面,因其利于开展,有很多优势,适合在临床中推广。

【Abstract】 Objective To compare the efficacy of stereotactic hematoma clearance and craniotomy in the treatment of cerebellar hemorrhage. Methods According to the set retrieval strategy,the major databases were searched extensively, and the included research was analyzed by meta with RevMan 5.3 software. Results A total of 591 cases were collected in 10 clinical studies, including 297 cases in the directional group and 294 cases in the craniotomy group. There was no significant difference between the two groups in postoperative daily activity and postoperative rebleeding(P >0.05).The directional group had more advantages in the operation and hospitalization time intraoperative bleeding volume and postoperative complications, the difference was statistically significant( P <0.05).Conclusion Frameless stereotactic hematoma removal, as a minimally invasive surgical method, has many advantages in the treatment of cerebellar hemorrhage, which is suitable for clinical application.
