声场分区辅助定位技术在甲状腺可疑微小癌细针穿刺活检中的应用研究

【摘要】 目的探讨声场分区辅助定位技术在甲状腺可疑微小癌细针穿刺活检(FNAB)中的临床应用价值。方法选取2018年6月至2020年12月于中山大学附属第五医院住院的可疑甲状腺微小癌患者,分为对照组与试验组,对照组采用徒手同平面内穿刺法,试验组采用橡皮筋声场分区辅助定位技术非同平面内穿刺法,比较两组的穿刺时间、穿刺次数、结节无法诊断率、恶性符合率与并发症发生率。结果对照组58例60个结节,试验组58例64个结节,两组间结节单次穿刺时间、无法诊断率、恶性符合率、并发症发生率等均无统计学差异(P> 0.05),而在穿刺次数及穿刺总时间上,试验组较对照组少,差异有统计学意义(P <0.05)。结论声场分区辅助定位非同平面内穿刺是超声引导下甲状腺结节FNAB的重要补充,对特殊部位的结节能增加穿刺成功率,缩短穿刺总时间,值得临床推广应用。

【Abstract】 Objective To evaluate the clinical value of sonic division assisted technique in fine needle aspiration biopsy(FNAB) of suspicious papillary thyroid microcarcinoma. Methods Patients with suspected papillary thyroid microcarcinoma were divided into control group and experimental group. The control group adopted the in-plane puncture method with bare hands, and the experimental group adopted the non-co-planar puncture method with rubber band sonic division assisted positioning technology. The puncture time, frequency, inadequate diagnostic rate, malignant coincidence rate, and complication rate were compared between the two groups. Results There were 60 nodules in 58 cases in the control group and 64 nodules in 58 cases in the experimental group. There were no significant differences in single puncture time, inadequate diagnostic rate, malignant coincidence rate, and complication rate between the two groups(P > 0.05), while the experimental group was less than the control group in the frequency and total time of puncture, and the difference was statistically significant(P < 0.05). Conclusions Sonic division assisted non-coplanar puncture is an important supplement to FNAB of thyroid nodules, which can increase the success rate of puncture and shorten the total puncture time.
