【摘要】 目的为了观察经气管镜下局部注射药物治疗气管内膜结核的临床疗效。方法选择我院近二年来(2018年1月至2020年1月)经电子支气管镜检查,最后确诊的气管内膜结核,共计48例,随机分为两组,治疗组:在全身化疗基础上,给予经电子支气管镜下局部注射抗结核药物治疗;对照组:在全身化疗基础上给予雾化治疗。密切观察两组病例临床症状、胸部影像变化以及纤维支气管镜下改变。结果治疗组与对照组比较,治疗2个月后临床症状变化差异有统计学意义(显效率χ~2=6.515,P<0.05;总有效率χ~2=7.111,P<0.01);治疗2个月后胸部影像变化差异有统计学意义(显效率χ~2=4.148,P<0.05;总有效率χ~2=4.0,P<0.05);治疗6个月后气管镜下改变比较差异有统计学意义(显效率χ~2=5.333,P<0.05;总有效率χ~2=5.4,P<0.05)。结论经电子支气管镜局部注药治疗气管内膜结核疗效显著,值得临床推广。更多还原

【Abstract】 Objective To observe the clinical efficacy of local injection of drugs under bronchoscope in the treatment of endotracheal tuberculosis.Methods A total of 48 cases of endotracheal tuberculosis diagnosed by electronic bronchoscopy in our hospital in recent two years (January 2018 to January 2020) were randomly divided into two groups:treatment group:on the basis of systemic chemotherapy,local injection of anti-tuberculosis drugs under electronic bronchoscope was given;Control group:Atomization treatment was given on the basis of systemic chemotherapy.Clinical symptoms,chest imaging changes and changes under fiberoptic bronchoscopy were observed closely.Results Compared with the control group,the changes of clinical symptoms in the treatment group were significantly different after 2 months of treatment (significant efficiency χ~2=6.515,P<0.05,Total effective rate χ~2=7.111,P<0.01).After 2 months of treatment,there was significant difference in chest imaging changes (significant efficiency χ~2=4.148,P<0.05.Total effective rate χ~2=4.0,P<0.05).After 6 months of treatment,there was significant difference in changes under bronchoscopy (significant efficiency χ~2=5.333,P<0.05.Total effective rate χ~2=5.4,P<0.05).Conclusion Local drug injection through electronic bronchoscope is effective in the treatment of endotracheal tuberculosis,which is worthy of clinical promotion.更多还原
