超声弹性成像参数在乳腺肿瘤诊断中的应用价值分析

【摘要】 目的探讨超声弹性成像参数在乳腺肿瘤诊断中的应用价值。方法回顾性分析2017年1月~2019年10月佛山市第二人民医院收治的60例乳腺肿瘤患者的临床资料,根据肿瘤的性质将31例良性肿瘤患者纳入A组,其余29例恶性肿瘤患者纳入B组,两组患者均予以超声检查。比较弹性成像和二维成像的临床应用价值。结果A组二维成像和弹性成像的直径和面积比较,差异无统计学意义(P>0.05);B组弹性成像的直径和面积大于二维成像,差异有统计学意义(P<0.05);B组弹性成像的直径和面积大于A组,差异有统计学意义(P<0.05);B组的直径变化率和面积比高于A组,差异有统计学意义(P<0.05);A组病灶软硬程度分值在1~5分的占比率(19.35%、29.03%、41.94%、9.68%和0.00%)与B组(0.00%、6.90%、13.79%、44.83%和34.48%)比较,差异有统计学意义(P<0.05);B组各项超声特征的发生率高于A组,差异有统计学意义(P<0.05)。结论对乳腺肿瘤患者使用超声弹性成像进行诊断可以客观反映肿瘤的相对硬度、病灶的典型特征以及面积并等定量参考指标,在判断病灶良性和恶性程度方面有一定优势,值得推广运用。

【Abstract】 Objective To explore the applied value of ultrasonic elastography parameters on diagnosis of breast tumor.Methods The clinical data about 60 breast tumor patients treated from January 2017 to October 2019 in the Second People′s Hospital of Foshan City was retrospectively analyzed. According to the nature of the tumor, thirty-one patients with benign tumor were included in the group A, and the remaining twenty-nine patients with malignant tumor were included in the group B. Both groups were examined by ultrasound. The clinical application value of elastic imaging and two-dimensional imaging were compared between the two groups. Results There were no significant differences in diameter and area of lesions between two-dimensional imaging and elastography imaging in group A(P>0.05). The diameter and area of lesions based on elastography imaging in group B was larger than those of two-dimensional imaging,the differences were significantly different(P<0.05). The diameter and area of lesions based on elastography imaging in group B was larger than those in group A, the differences were significantly different(P<0.05). The diameter change rate and area ratio of group B were higher than that of group A, the differences were significantly different(P<0.05).Compared the ratio of patients from grade 1 to grade 5 of tumor hardness for group A(19.35%, 29.03%, 41.94%,9.68% and 0.00%) and group B(0.00%, 6.90%, 13.79%, 44.83% and 34.48%), the differences were significantly different(P<0.05). The incidence of typical ultrasonic features in group B was higher than that in group A, the differences were significantly different(P<0.05). Conclusion The ultrasonic elastography parameters are used to quantitatively determine the tumor hardness, typical features and area, which plays a significant role in the differential diagnosis of benign and malignant tumor. It is worthy of clinical promotion and application.
