血清胃泌素与2型糖尿病患者血糖水平的相关性分析

【摘要】 目的探讨血清胃泌素(Gas)水平与T2DM患者血糖控制情况的相关性。方法选取2017年1月至2019年12月于湘雅二医院代谢内分泌科住院的T2DM患者185例,分为HbA1c<7%组(n=110)及HbA1c≥7%组(n=75),分析Gas水平与HbA1c、FPG、TC、LDL-C、HDL-C、Ins使用率、降糖药物使用率的相关性。结果 HbA1c<7%组FPG低于HbA1c≥7%组,血清Gas-17、Ins使用率等指标高于HbA1c≥7%组(P<0.05或P<0.01)。两组BMI、DM病程、SBP、DBP、TC、TG、LDL-C、HDL-C等差异均无统计学意义(P>0.05)。Spearman相关性分析显示,血清Gas-17与FPG(r=-0.318,P<0.01)及HbA1c(r=-0.507,P<0.01)呈负相关。Logistic多元线性回归分析结果显示,HbA1c是Gas-17的独立影响因素。结论 T2DM患者血清Gas水平与血糖控制呈负相关,对血糖控制的评估起重要作用。

【Abstract】 Objective To investigate the relationship between blood glucose(BG)control and serum gastrin(Gas)levels in patients with type 2 diabetes mellitus(T2 DM).Methods A total of 185 inpatients with T2 DM in the Second Xiangya hospital from January 2017 to December 2019 were collected and divided into two groups based on the glycosylated hemoglobin(HbA1c),HbA1c<7% and HbA1c≥7%.The relationship between serum Gas-17 levels and HbA1c,FPG,TC,LDL-CHDL-C,insulin usage and hypoglycemic drug usage.Results Compared with HbA1c ≥7% group,patients in HbA1c <7% group had significantly lower FPG,and higher serum Gas-17 and insulin usage rate(P<0. 05 or P<0. 01). There were no significant differences in BMI,diabetes course,SBP,DBP,TC,TG,LDL-C,HDL-C and other indicators between the two groups(P>0. 05). Spearman correlation analysis showed that serum Gas-17 level was negatively correlated with FPG(r=-0. 318,P<0. 00)and HbA1c(r=-0. 507,P=0. 000). Multiple linear regression analysis showed that HbA1c was an independent predictor of gastrin secretion.Conclusion Serum gastrin in patients with T2 DM is negatively correlated with BG control,which may have a certain effect on the evaluation of BG management.
