【摘要】 背景:应用Ilizarov外固定器治疗膝关节僵直常造成膝脱位及膝周韧带损伤,有待改装较为合适的连接配件进行治疗。目的:探索Ilizarov外固定器加装弹簧式铰链治疗创伤性膝关节僵直的可行性及临床效果。方法:回顾性分析2017年2月至2019年9月采用Ilizarov外固定器加装弹簧治疗12例创伤性膝关节僵直患者的临床资料,男7例,女5例;年龄21～48岁,平均(30.1±8.7)岁。僵直状态:伸直位2例,轻度屈膝位7例,半屈膝位2例,伸直伴膝外翻位1例。缓慢调整加装弹簧的Ilizarov外固定器,完成2～3次屈膝牵伸治疗,术后患膝配合功能康复锻炼3～6个月。结果:随访时间6～18个月,平均(10.4±7.5)个月。12例患者顺利完成至少2次屈膝牵伸治疗。6例患者膝屈伸活动接近70°,4例患者膝屈伸活动度接近90°,2例膝屈伸活动度超过100°。结论:利用弹簧式铰链Ilizarov外固定器治疗创伤性膝关节僵直的牵伸力持续而不易损伤膝关节,使膝关节僵直得到有效治疗,临床疗效满意。更多还原

【Abstract】 Background: Ilizarov external fixator usually causes knee dislocation and injury of ligament around knee in the treatment of knee stiffness which should be improved with suitable connection accessories. Objective: To explore the feasibility of Ilizarov external fixator with spring hinge in the treatment of traumatic knee stiffness. Methods: Totally 12 patients with traumatic knee stiffness treated by Ilizarov external fixator with spring hinge from February 2017 to September 2019 were included in the retrospective study including 7 males and 5 females, with the mean age of(30.1±8.7) years(range, 21-48 years). According to rigid state, there were 2 patients in extension position, 7 patients in in mild flexion position, 2 patients in semi flexion position and 1 patient in valgus position. Ilizarov external fixator with spring hinge was slowly adjusted to complete 2-3 times of knee distraction treatment. The patients took function rehabilitation exercise for 3-6 months with affected knee after the operation. Results: All patients were followed up for 6-18 months, with the mean of(10.4±7.5) months. All patients successfully completed at least two treatments of knee distraction. Six patients had knee flexion range of about 70°; 4 patients had knee flexion range of about 90°; 2 patients had knee flexion range over 100°. Conclusions: Ilizarov external fixator with spring hinge is effective in clinical practice and is worthy of clinical promotion, for it can make continuous knee distraction with less injury of knee.更多还原
