【摘要】 目的观察光动力疗法（Photodynamic therapy,PDT）联合超脉冲CO₂点阵激光治疗重度痤疮的临床疗效。方法选择2018年1月至2019年11月,重度痤疮患者40例,随机分为治疗组和对照组,每组患者各20例。（1）治疗组,采用PDT联合超脉冲CO₂点阵激光治疗,先行CO₂激光针对脓肿、结节采用连续模式治疗,功率2 W,每点间隔2 mm,多点打孔引流出脓性分泌物后,再采用点阵模式,能量120 m J,间距1.2 mm,覆盖率4%,建立通道有利于脓液排出和促进光敏剂吸收。CO₂激光打孔后,外敷光敏药物5-氨基酮戊酸甲脂2 h后,进行PDT治疗,波长（633±5）nm光照射,时间20 min,能量密度150～200m J/cm²。（2）对照组,仅采用PDT治疗,照射时间20 min,能量密度150～200 m J/cm²。两组患者均每周治疗1次,连续治疗3次。每次治疗前和治疗后1周两组均采集图像进行临床疗效比较,并记录治疗后的不良反应。结果 PDT联合超脉冲CO₂点阵激光治疗组的疗效优于对照组（P<0.05）。两组患者3次治疗后1周,治疗组有效率（90.0%）高于对照组有效率（60.0%）,差异具有统计学意义（P<0.05）。两组均无严重不良反应发生。结论 PDT联合超脉冲CO₂点阵激光治疗重度痤疮治愈率高,不良反应少,具有很好的临床应用前景。更多还原

【Abstract】 Objective To observe the clinical efficacy to treat severe acnes jointly with photodynamic therapy and fractional ultrapulsed CO₂ laser.Methods Totally 40 cases of severe acnes were randomly divided into two groups from Jan.2018 to Nov.2019:the treatment group（20 cases）,in which photodynamic therapy and fractional ultrapulsed CO₂ laser were applied jointly,and the control group（20 cases）,in which photodynamic therapy was used alone.The treatment was given once a week for 3 times.Before each treatment,images were collected to compare the clinical efficacy of the two groups,and the adverse reactions after the treatment were recorded.Results The therapeutic effect of the combination of photodynamic therapy and fractional ultrapulsed CO₂ laser was better than that of the sole application of photodynamic therapy.After 3 times treatment,the difference between two groups was statistically significant.No serious adverse reaction was found in either groups.Conclusions Featuring high cure rate and less adverse reactions,the combination of photodynamic therapy and fractional ultrapulsed CO₂ laser has a good clinical application prospect in treating severe acnes.更多还原
