心脏超声与术中胸片定位儿童输液港导管尖端的对比研究
Comparative Study of Cardiac Ultrasound and Intraoperative Chest Radiograph To Position the Tip of Implantable Venous Access Port in Children
【摘要】 目的总结心脏超声定位导管尖端在儿童输液港植入手术中的应用体会,对比心脏超声与X-ray两种方法定位输液港导管尖端在手术时间、术后并发症等方面的差异性,了解心脏超声定位导管尖端在儿童植入式输液港手术中的应用的安全性与可行性。方法将100例植入式输液港的儿童病例随机分成两组,其中心脏超声定位导管尖端(超声组)50例,X-ray定位导管尖端(胸片组)50例,所有患儿均在超声引导下行颈内静脉穿刺置管,比较两组在手术时间、术中出血、术后导管感染、导管血栓、尖端移位的差异性。结果超声组在手术时间和术中出血方面明显少于胸片组,差异有统计学意义(P<0.05);超声组术后并发症数量少于胸片组。结论心脏超声定位输液港导管尖端在儿童中的应用更加精准、省时,值得临床上积极广泛的推广使用。
【Abstract】 Objective To summarize the application experience of the tip of the cardiac ultrasound positioning catheter in the operation of the implantable infusion venous access port in children. Comparing the difference of the two methods of cardiac ultrasound and X-ray in the operation time and postoperative complications to understand the safety and feasibility of the application of ascertain catheter tip by cardiac ultrasound in the operation of implantable venous access port in children. Methods 100 child cases underwent implantable venous access port implantation were randomly divided into two groups, 50 cases for each of the ultrasound positioning catheter tip(ultrasound group) and the X-ray positioning catheter tip(chest radiography group). The ultrasound group and the chest radiography group were placed the catheters by ultrasound-guided and compared the otherness in operation time, intraoperative bleeding, postoperative catheter infection, catheter thrombosis, and tip displacement between two groups. Results The ultrasound group was significantly less than the chest radiography group in terms of operation time and intraoperative bleeding, the difference was statistically significant(P<0.05). The number of postoperative complications in ultrasound group was less than that in chest radiograph group. Conclusions The application of cardiac ultrasound positioning catheter tip in implantable venous access port for children is more accurate and time-saving so it is worthy of active and extensive clinical promotion.
