弹性成像联合超声造影定量评估颈动脉低回声斑块与脑梗死再发的关系

【摘要】 目的应用超声弹性成像结合超声造影技术定量分析颈动脉低回声斑块对脑梗死再发的预测价值。方法选取87例临床首次诊断为脑梗死并彩超常规证实有颈动脉低回声斑块的患者,均行超声弹性成像及超声造影,随访一年,分析颈动脉低回声斑块与脑梗死再发的关系。结果 (1)脑梗死再发组Emean、Emax、Emin均低于脑梗死未再发组,而脑梗死再发组PI高于脑梗死未再发组(P<0.05);(2)ROC分析显示,以PI≥20.99 dB且Emax≤21.20 kPa为最佳截断值联合预测脑梗死再发的曲线下面积为0.922(95%CI,0.863~0.982,P<0.001),灵敏度和特异度分别为88.0%、87.0%。结论弹性成像及超声造影技术联合定量评估颈动脉低回声斑块能明显提高脑梗死再发的预测价值。

【Abstract】 Objective To evaluate quantitatively the utility of the prediction of carotid hypoechoic plaque using elastography ultrasound and contrast enhanced ultrasonography in recurrent cerebral infarction. Methods Eighty-seven subjects with first cerebral infarction and carotid hypoechoic plaque by conventional ultrasound were underwent elastography and contrast enhanced ultrasonography. The condition of recurrent cerebral infarction in next year was followed up. The relationship between carotid hypoechoic plaque and cerebral infarction was analyzed. Results(1)In the recurrent cerebral infarction group, Emean,Emax and Emin were lower than those in the non recurrent cerebral infarction group, while PI was higher than that of the non recurrent cerebral infarction group;(2)According to ROC curve analysis,the sensitivity and specificity in prediction of recurrent cerebral infarction by PI(cut off value≥20.99 dB) and Emax(cut off value≤21.20 kPa) were 88.0% and 87.0% respectively and the area under ROC curve was 0.922(95%CI,0.863-0.982,P<0.001). Conclusions The predictive value of recurrent cerebral infarction will see evident increase when elastography ultrasound and contrast enhanced ultrasonography evaluate quantitatively carotid hypoechoic plaque.
