子宫内膜容受性标志物与行IVF-ET反复种植失败的相关性研究

【摘要】 目的探讨子宫内膜容受性(Endometrial receptivity,ER)标志物整合素β1、整合素β3、Rac1、HoxA11、Noggin在增生期及种植窗期的表达及与反复种植失败(Repeated implantation failure,RIF)的相关性。方法收集淮安市妇幼保健院2018年1月~2020年12月行体外受精-胚胎移植(In vitro fertilization-embryo transfer,IVF-ET)治疗的140例患者的增生期及种植窗期内膜组织。采用酶联免疫吸附(Enzyme linked immunosorbent assay,ELISA)法检测RIF患者(RIF组,n=80)和成功妊娠患者(对照组,n=60)整合素β1、整合素β3、Rac1、HoxA11、Noggin在增生期及种植窗期的表达水平。采用Spearman相关性分析种植窗期整合素β1、整合素β3、Rac1、HoxA11、Noggin与RIF的关系。结果增生期,RIF组ER标志物整合素β1水平显著高于对照组(P <0.05),Noggin水平显著低于对照组(P <0.05)。种植窗期,RIF组ER标志物整合素β1水平显著高于对照组(P <0.05),整合素β3、Rac1、HoxA11、Noggin水平显著低于对照组(P <0.05)。种植窗期各组ER标志物整合素β1、整合素β3、Rac1、HoxA11、Noggin水平均显著高于增生期(P <0.05)。Spearman相关性分析结果显示,种植窗期整合素β1水平与RIF呈正相关(r=0.615,P=0.021),整合素β3、HoxA11水平与RIF呈负相关(r=-0.599,P=0.032;r=-0.732,P=0.013),Rac1、Noggin水平与RIF无相关性(r=-0.412,P=0.063;r=-0.357,P=0.081)。结论整合素β1、整合素β3、Rac1、HoxA11、Noggin在增生期及种植窗期均有表达。种植窗期子宫内膜整合素β1表达水平升高和整合素β3、HoxA11表达水平降低与RIF发生密切相关。

【Abstract】 Objective To investigate the expression of ER markers integrin β1, integrin β3, Rac1, Hox A11 and Noggin in proliferative stage and implantation window stage and their correlation with RIF. Methods 140 cases of IVF-ET endometrial tissues in proliferative stage and implantation window stage were collected from Huai’an maternal and child health hospital from Jan 2018 to Dec 2020. The expression levels of integrin β1, integrin β3, Rac1, HoxA11 and Noggin in patients with RIF(RIF group, n = 80) and patients with successful pregnancy(control group, n = 60) were detected by enzyme linked immunosorbent assay(ELISA).Spearman correlation was used to analyze the relationship between integrin β1, integrin β3, Rac1, HoxA11, Noggin and RIF. Results In proliferative stage, the level of ER marker integrin β1 in RIF group was significantly higher than that in the control group(P < 0.05), and the level of Noggin in RIF group was significantly lower than that in the control group(P < 0.05). At the implantation window stage, the level of ER marker integrin β1 in RIF group was significantly higher than that in the control group(P < 0.05), and the levels of integrin β3, Rac1, HoxA11 and Noggin in RIF group were significantly lower than those in the control group(P< 0.05). The levels of ER markers integrin β1, integrin β3, Rac1, HoxA11 and Noggin in the implantation window stage were significantly higher than those in the proliferative stage(P < 0.05). Spearman correlation analysis showed that integrin β1 level was positively correlated with RIF(r = 0.615, P = 0.021), integrin β3 and HOXA11 levels were negatively correlated with RIF(r =-0.599, P = 0.032; r =-0.732, P = 0.013), Rac1 and Noggin levels were not correlated with RIF(r =-0.412, P = 0.063; r =-0.357, P = 0.081). Conclusion Integrin β1, integrin β3, Rac1, HoxA11 and Noggin are expressed in proliferative stage and planting window stage. The increase of integrin β1 and the decrease of integrin β3 and HOXA11 in the implantation window are closely related to RIF.
