宫腔镜术后盆腔感染的病原菌分布及其耐药性分析

【摘要】 目的探究宫腔镜术后盆腔感染的病原菌分布及耐药性分析,为临床抗感染药物的选择提供依据。方法研究选取2016年3月~2019年12月进行宫腔镜手术的患者120例,无菌操作下取患者盆腔分泌物进行培养和细菌鉴定,对细菌的药敏性进行研究分析。结果 120例患者行宫腔镜术后发生盆腔感染12例,感染率为10.00%,共检出病原菌26株,革兰阳性菌13株,占50%,其中主要以表皮葡萄球菌与金黄色球菌为主,分别占19.23%,11.54%;革兰阴性菌13株,占50.00%,其中主要以大肠埃希菌及肺炎克雷伯菌为主,分别占23.08%、16.00%。革兰阴性菌中大肠埃希菌及肺炎克雷伯菌对头孢唑林、阿莫西林、头孢曲松、氨苄西林具有较高的耐药性,而对庆大霉素、阿卡米星、美罗培南的耐药性较低。革兰阳性菌中表皮葡萄球菌及金黄色葡萄球菌对青霉素、罗红霉素、阿奇霉素等具有较高的耐药性,对亚胺培南、替考拉宁、利奈唑胺的耐药性低。结论宫腔镜术后盆腔感染的病原菌种类较多,不同的病原菌对抗感染药物会表现出不同的耐药性,因此临床治疗时应根据药敏试验的结果合理选择药物治疗,提高抗感染治疗的效果。

【Abstract】 Objective To investigate the pathogenic bacteria distribution and drug resistance analysis of pelvic infection after hysteroscopic surgery, and provide evidence for the selection of clinical anti-infective drugs. Methods The study selected 120 patients who underwent hysteroscopic surgery in our hospital from March 2016 to Dec ember 2019. The pelvic secretions were taken for culture and bacterial identification under aseptic operation, and the drug sensitivity of the bacteria was studied and analyzed. Results In 120 patients, 12 cases of pelvic infection occurred after hysteroscopic surgery, the infection rate was 10.00%, and 26 pathogens and 13 gram-positive bacteria were detected, accounting for 50%, mainly Staphylococcus epidermidis and Staphylococcus aureus. accounting for 19.23%, 11.54%, respectively, 13 gram-negative bacteria, accounting for 50.00%, mainly Escherichia coli and Klebsiella pneumoniae, accounting for 23.08%, 16.00%. Escherichia coli and Klebsiella pneumoniae in gram-negative bacteria have high resistance to cefazolin, amoxicillin, ceftriaxone, and ampicillin, but to gentamicin, aqamilcin, meropenem is less resistant. Staphylococcus aureus and Staphylococcus aureus in gram-positive bacteria have high resistance to penicillin, roxithromycin, azithromycin, etc., and low resistance to imipenem, teicoplanin and linezolid. Conclusion There are many types of pathogens in pelvic infection after hysteroscopic surgery. Different pathogens will show different drug resistance against infectious drugs. Therefore, according to the results of drug sensitivity test, drug therapy should be rationally selected in clinical treatment to improve the effect of anti-infective treatment.
