教师扮演标准化病人在医学本科生病历书写教学中的应用
The Application of Teachers Acting as Standardized Patients in Medical Undergraduates Medical Record Writing Teaching
【摘要】 目的探讨教师扮演标准化病人在本科生病历书写教学中的作用,从而提高临床病历书写教学质量。方法将164名某校2017级五年制临床医学专业本科生分为试验组和对照组,每组各82人,于2020年10月1日-2020年11月30日进行妇产科病历书写教学,试验组和对照组由同一个见习教师进行病历书写的理论知识教学,试验组学生的病历书写培训由教师扮演标准化病人配合,对照组的由住院患者配合。两组学生书写的病历按照统一标准进行评分。对两组学生进行无记名问卷调查。结果试验组学生的病例书写评分显著高于对照组学生(88.49±2.99分vs 85.11±3.24分),差异具有统计学意义(P=0.00);实验组在提高学生的学习积极性(P=0.02)、掌握理论知识(P=0.00)和培养临床思维能力方面(P=0.01)都显著优于对照组,但在提高学生医患沟通能力方面,2组无显著差异(P=0.31)。结论教师扮演标准化病人有助于本科学生病历书写培训,但需要进一步完善以提高学生对该模式的兴趣,同时提高对学生医患沟通能力的培养,值得在妇产科教学中推广。
【Abstract】 Objectives To explore the role of teachers playing standardized patients in the teaching of medical record writing for undergraduates, so as to improve the teaching quality of clinical medical record writing. Methods 164 cases of five-year clinical medicine undergraduates from a school in 2017 were divided into the experimental group and the control group, with 82 in each group. Medical record writing teaching of obstetrics and gynecology was conducted from October 1 st, 2020 to November 30 th, 2020. The theoretical knowledge of medical record writing was taught by the same student teacher in the experimental group and the control group. In the test group, the teacher acted as the standardized patient, and in the control group, the inpatient cooperated. The medical records written by students of both groups were graded according to uniform standards. The two groups of students were investigated by anonymous questionnaire. Results The scores of writing medical record of the students from the experimental group were significantly higher than those from the control group(88.49±2.99 vs 85.11±3.24, P<0.05). Compared with the traditional teaching model, teachers played standardized patients was significantly better to improve students’ learning enthusiasm(P=0.02), was more helpful to master students’ theoretical knowledge(P=0.00) and cultivate students’ clinical thinking ability(P=0.01). However, there was no significant difference between the two groups in improving doctor-patient communication skills(P>0.05). Conclusions Teachers playing standardized patients was helpful for undergraduate students’ medical record writing training, but it needed to be further improved to improve students’ interest in this mode and improve students’ doctor-patient communication ability. It was worth promoting in obstetrics and gynecology teaching.
