【摘要】 目的探讨黄葵胶囊联合辛伐他汀治疗2型糖尿病肾病(DKD)的临床效果及对细胞因子等的影响。方法选择2017年10月～2019年10月金华广福医院及金华职业技术学院医学院门诊收治的116例2型DKD患者,随机分为观察组58例和对照组58例,两组均给予常规治疗,对照组口服辛伐他汀40 mg 1次/d,晚上顿服。观察组联合黄葵胶囊2.5 g,3次/d口服。12周为1疗程。观察两组治疗前后血糖、血脂代谢水平、肾功能、细胞因子及肾纤维化指标等。结果观察组治疗后总胆固醇(TC)、三酰甘油(TG)、糖化血红蛋白(HbA1c)、空腹血糖(FPG)水平均明显低于对照组(P均<0.01);观察组治疗后肿瘤坏死因子-α(TNF-α)、转化生长因子β1(TGF-β1)、白介素-6(IL-6)均明显低于对照组(P<0.01);观察组治疗后基质金属蛋白酶-9(MMP-9)水平明显高于对照组(P<0.01),血尿素氮(BUN)、血肌酐(SCr)、尿微量白蛋白排泄率(UAER)水平和金属蛋白酶抑制剂-1(TIMP-1)水平均明显低于对照组(P均<0.01)。结论黄葵胶囊联合辛伐他汀治疗2型DKD可延缓肾脏纤维化,改善肾功能,减少尿蛋白排泄率,其机制可能与有效改善脂质代谢紊乱、调节细胞因子失衡等有关。更多还原

【Abstract】 Objective To investigate the clinical effect of huangkui capsule combined with simvastatin in the treatment of type 2 diabetes kidney disease(DKD) and its influence on cytokines. Methods A total of 116 patients with type 2 DKD admitted to Jinhua Guangfu Tumor Hospital and Medical College of Jinhua Polytechnic College from October2017 to October 2019 were randomly divided into the observation group(58 cases) and the control group(58 cases).Both groups were given routine treatment. The control group was given 40 mg simvastatin once a day at night. The observation group was administrated orally with 2.5 g huangkui capsule on the basis of the control group, 3 times per day.Both group were treated for 12 weeks as a course of treatment. Blood glucose, blood lipid metabolism, renal function,cytokines and renal fibrosis indexes were observed before and after treatment in the two groups. Results The levels of total cholesterol(TC), triacylglycerol(TG), glycosylated hemoglobin(HbA1 c) and fasting blood glucose(FPG) were significantly lower in the observation group than those in the control group after treatment(P<0.01). Tumor necrosis factor(TNF), transformation growth factor 1(TGF-1) and interleukin-6(IL-6) were significantly lower in the observation group than those in the control group after treatment(P<0.01). The level of matrix metalloproteinase-9(MMP-9) was significantly higher in the observation group than that in the control group after treatment(P<0.01), while the levels of blood urea nitrogen(BUN), creatinine(SCr), urinary microalbumin excretion rate(UAER) and inhibitor of metalloproteinase-1(TIMP-1) were significantly lower in the observation group than those in the control group(P<0.01). Conclusion Huangkui capsule combined with simvastatin can delay renal fibrosis, improve renal function and reduce urinary protein excretion rate of patients with type 2 DKD. Its mechanism may be related to effectively improving lipid metabolism disorder and regulating cytokine imbalance.更多还原
