MTHFR C677T基因多态性与浙江地区汉族人群大动脉粥样硬化性脑梗死的相关性
Correlation between MTHFR C677T gene polymorphism and large artery atherosclerotic cerebral infarction in the population of Han ethnicity in Zhejiang region
【摘要】 目的探讨MTHFR C677T基因多态性与浙江地区汉族人群大动脉粥样硬化性(LAA)脑梗死的相关性。方法选取2017年1月至2018年6月在我院住院患者及同期体检人员共360例,分为LAA脑梗死组(病例组)180例和对照组180例,应用PCR及TaqMan探针技术对MTHFR C677T进行基因分型检测,分析该基因多态性与LAA脑梗死的相关性。结果病例组和对照组间的SBP、FBG、HbA1c、TC、TG、叶酸、Hcy比较,差异均有统计学意义(P<0.05);两组间的年龄、DBP、LDL-C、HDL-C比较,差异均无统计学意义(P>0.05)。两组间的基因型和等位基因频率分布比较,差异有统计学意义(P<0.05)。病例组TT型Hcy水平为[(14.10±3.96)μmol/L]高于CC型的[(9.81±3.04)μmol/L]及CT型的[(11.49±3.22)μmol/L],对照组TT型Hcy水平为[(12.36±3.62)μmol/L]高于CC型的[(9.26±2.82)μmol/L]及CT型的[(10.15±3.04)μmol/L]。单因素线性回归分析显示,等位基因T与叶酸(r=0.261,β=-1.188,t=-4.847,P<0.001)、Hcy(r=0.317,β=2.404,t=6.327,P<0.001)、FBG(r=0.106,β=0.435,t=1.985,P=0.048)存在线性回归关系。Logistic回归分析示等位基因T、高血压、糖尿病、Hcy是LAA脑梗死的独立危险因素(P<0.05)。结论 MTHFR C677T基因多态性影响Hcy、FBG,是浙江地区汉族人群LAA脑梗死的危险因素。
【Abstract】 Objective To investigate the correlation between the gene polymorphism of methylenetetrahydrofolate reductase(MTHFR) C677T and large artery atherosclerosis(LAA) cerebral infarction in the population of Han ethnicity in Zhejiang Region. Methods From January 2017 to June 2018, a total of 360 inpatients and people receiving physical examination during the same period of time from Taizhou Municipal Hospital were selected. They were divided into 180 cases in LAA cerebral infarction group(case group) and 180 cases in control group. The PCR and TaqMan probe technology were applied for the genotyping of MTHFR C677T, and the correlation between this gene polymorphism and LAA cerebral infarction was analyzed. Results The differences in SBP, FBG, HbA1c, TC, TG, folic acid and Hcy between the case group and the control group were statistically significant(P<0.05). There was no statistically significant difference in age, DBP, LDL-C and HDL-C between the two groups(P>0.05). The difference in genotype and allele frequency distribution between the two groups was statistically significant(P<0.05). The TT type Hcy level in the case group([14.10±3.96] μmol/L) was higher than that of CC type([9.81±3.04] μmol/L) and CT type([11.49±3.22] μmol/L). The TT type Hcy level([12.36±3.62] μmol/L) in the control group was higher than that of CC type([9.26±2.82] μmol/L) and CT type([10.15±3.04] μmol/L). Single factor linear regression analysis showed that allele T and folic acid(r=0.261, β=-1.188, t=-4.847, P<0.001), Hcy(r=0.317, β=2.404, t=6.327, P<0.001), and FBG(r=0.106, β=0.435, t=1.985, P=0.048) had a linear regression relationship. Logistic regression analysis showed that allele T, hypertension, diabetes and HHcy were independent risk factors for LAA cerebral infarction(P<0.05). Conclusion MTHFR C677T gene polymorphism affects Hcy and FBG and is a risk factor for LAA cerebral infarction in the population of Han ethnicity in Zhejiang region.
