膳食酸负荷与中老年人骨密度的关系

【摘要】 目的探讨膳食酸负荷与中老年人骨密度的关系。方法研究对象来自于"广州营养与健康研究"队列人群,选择资料完整的3319名40~75岁的居民,其于2011~2013年完成了食物频数问卷、一般情况调查和骨密度的测定(全身、腰椎、髋骨、转子间和股骨颈)。利用膳食资料计算了膳食酸负荷--潜在肾酸负荷(PRAL)和内源性酸净生成量(NEAP),分析膳食酸负荷与骨密度间的关系。结果校正潜在干扰因素后,女性膳食酸负荷越高,其腰椎L1-L4、总髋骨、转子间和股骨颈的骨密度越低。且其总髋骨、转子间的骨密度值在PRAL的四分位组最高组比最低组分别低1.85%、2.46%(~*P<0.05),男性虽然也呈类似趋势,但多因素校正后的关联均无统计学意义。;分层分析中,年龄大于60岁的女性其转子间的骨密度也会随着PRAL的升高而降低。结论膳食酸负荷与中老年妇女骨密度成负相关。[营养学报,2021,43(1):36-42]

【Abstract】 Objective To examine the association between dietary acid load and bone mineral density in middle-aged and elderly people. Methods The subjects were selected from a cohort study in Guangzhou Nutrition and Health Study in which 3319 participants completed the food frequency questionnaire, general survey and bone mineral density(BMD) measurement in the whole body, lumbar, innominate, intertrochanteric and femoral neck bone during 2011-2013. Dietary acid load, potential renal acid load(PRAL) and net endogenous acid production(NEAP) were calculated according to the dietary data. The relationship between BMD and PRAL or NEAP was analyzed. Results After adjusting for potential covariates, higher levels of PRAL and NEAP were associated with lower BMD in the lumbar L1-L4, total innominate, intertrochanteric and femoral neck bone among women. The BMD values of total innominate, intertrochanteric were 1.85%, 2.46% lower in the highest(vs. lowest) quartile of PRAL respectively, indicating lower BMD with greater dietary acid load. Although there was a similar trend in the males, the multivariate-adjusted associations were not statistically significant. However, in the stratified analysis, we found that the intertrochanteric bone density of women older than 60 years also decreased with the increase of PRAL. Conclusion Dietary acid load is inversely associated with BMD in middle-aged and elderly women, but not in men. [ACTA NUTRIMENTA SINICA, 2021, 43(1):36-42]
