【摘要】 惰性病变与医学干预问题涉及生物医学、公共卫生和社会经济诸多方面,是近年来颇受关注的研究领域。从一个病理医生的角度,探讨了肿瘤性惰性病变的概念和临床意义,介绍了上皮来源惰性病变的定义和应用。从惰性和进展、局部和整体及社会环境对惰性病变的生物学性质影响进行了探讨。介绍了癌症筛查及临床实践中惰性病变的过度诊断和过度治疗的现状并分析了可能原因。建议客观全面认识惰性病变的生物学性质,结合我国国情,密切随诊随访和相关研究工作,创建精准恰当的惰性病变医学干预体系。更多还原

【Abstract】 The study of indolent lesions and medical interventions involves biomedicine, public health and socio-economic development, which has aroused extensive concern in recent decades. This paper explores the conception and clinical significance of neoplastic indolent lesions and medical interventions from the perspective of pathologists, especially the definition and application of indolent lesion of epithelial origin(IDLE). The biological nature of neoplastic indolent lesions is analyzed with respect to indolence vs. progression, local lesion vs. systemic and environmental influence. The status and possible causes of over diagnosis and overtreatment in cancer screen and clinical practice are discussed. In the light of actual conditions of China, it is suggested to objectively understand the biological natures of indolent lesions, keep close follow-up, and establish accurate medical intervention system.更多还原
