【摘要】 目的探究降钙素原(PCT)与C-反应蛋白(CRP)用于经皮肾镜取石术(PCNL)患者术后感染诊断价值。方法选取2017年1月至2019年12月来本院泌尿外科行PCNL治疗的患者102例,按照是否发生术后感染分为感染组31例和非感染组71例,检测入组对象的PCT、CRP水平,并运用受试者工作曲线(ROC)进行PCNL术后感染诊断效能的分析,确定最佳诊断临界值。结果感染组患者的PCT、CRP水平均显著性高于非感染组患者差异有统计学意义(P<0.05);ROC曲线显示,PCT的诊断最佳临界值为0.56μg/L,灵敏度及特异度分别为77.4%和93.00%,约登指数为0.704,AUC为0.821;CRP的诊断最佳临界值为6.35 mg/L,灵敏度及特异度分别为93.5%和97.2%,约登指数为0.907,AUC为0.987;PCT、CRP两者联合检测灵敏度及特异度分别为96.80%和100.00%,约登指数为0.968,AUC为0.998。结论行PCNL治疗发生术后感染的患者的PCT、CRP水平升高,二者联合检测对PCNL术后感染具有较高的诊断价值。更多还原

【Abstract】 Objective To explore the diagnostic value of procalcitonin(PCT)and C-reactive protein(CRP)in the diagnosis of postoperative infection in patients with percutaneous nephrolithotomy(PCNL).Methods A total of 102 patients who came to our Urology Department for PCNL treatment from January2017 to December 2019 were selected and divided into the infection group(31 cases)and the non-infection group(71 cases)according to whether postoperative infection occurred. The PCT and CRP levels of the enrolled subjects were detected,and the receiver operating curve(ROC)was used to analyze the diagnostic efficacy of infection after PCNL to determine the best diagnostic cut-off value. Results The levels of PCT and CRP in the infected group were significantly higher than those in the non-infected group(P<0.05). The ROC curve shows that the optimal cutoff value for PCT diagnosis is 0.56 μg/L,the sensitivity and specificity are77.4% and 93.00%,respectively,the Youden index is 0.704,and the AUC is 0.821.The best critical value for CRP diagnosis is 6.35 mg/L,the sensitivity and specificity are 93.5% and 97.2%,respectively,the Youden index is 0.907,and the AUC is 0.987. The sensitivity and specificity of combined detection of PCT and CRP were 96.80% and 100.00%,respectively,the Youden index was 0.968,and the AUC was 0.998. Conclusion The levels of PCT and CRP in patients undergoing PCNL treatment for postoperative infection are elevated,and the combined detection of the two has a high diagnostic value for postoperative infection of PCNL.更多还原
