【摘要】 目的探讨经胸腔镜手术治疗新生儿先天性膈疝的手术方法及临床经验。方法回顾分析我院2018年3月-2020年3月收治的21例新生儿先天性膈疝患儿的临床资料,其中男性12例,女性9例,年龄26h～15d,左侧膈疝19例,右侧膈疝2例,术前均经胸片、上消化道造影获得诊断,均行胸腔镜下膈肌修补术。结果 20例患儿顺利完成胸腔镜手术,手术时间为95～168min,平均为(128±13) min;术中出血量为3.0～8.0mL,平均为(3.0±1.2) m L。术后需呼吸机支持天数为3.0～12.0d,平均为(4.9±1.2) d。1例患儿中转开放手术,其余患儿均治愈出院。结论经胸腔镜手术治疗先天性膈疝术野显露清楚、操作方便、损伤小、切口美观,疗效满意。更多还原

【Abstract】 Objective To explore the efficacy of thoracoscopic surgery for repairing congenital diaphragmatic hernia(CDH) in newborns and summarize the relevant clinical experiences. Methods Collecting clinical data of 21 neonates with CDH underwent thoracoscopie surgery from March 2018 to March 2020.Among them,there were twelve males and nine females at the age between 26 hours to 15 days old.The diaphragmatic hernia included on the left ones with 19 cases(n=19)and the right ones with 2 cases(n=2).All preoperative diagnoses were made by chest radiography, upper gastrointestinal radiography, and all underwent thoracoscopic diaphragmatic repair.Results Thoracoscopic surgery was successfully performed with 20 cases.The operative time was 95-168 min.The average was(128±13)min,the blood loss was(3.0-8.0)mL and the average was(3.0±1.2)mL,and the postoperative mechanical ventilation time was(3.0-12.0)d and the average was(4.9±1.2)d. One child was transferred to open surgery while the rest of the patients were cured and discharged from hospital.Conclusions Thoraeoseopic repair of congenital diaphragmatic herniais was safe and effective.The surgical view was clear.The operation was convenient.The injury was light.The incision was nice and the therapeutic effect was satisfactory.更多还原
