康艾注射液联合贝伐单抗治疗对晚期非小细胞肺癌患者循环肿瘤细胞及CA21-1水平的影响

【摘要】 目的观察康艾注射液联合贝伐单抗治疗对晚期非小细胞肺癌(NSCLC)患者循环肿瘤细胞(CTC)及细胞角蛋白19片段(CA21-1)水平的影响。方法选取2018年6月—2020年6月河北省胸科医院肺肿瘤三科收治晚期NSCLC患者96例,根据随机数字表法分为对照组(n=43)和观察组(n=53)。对照组给予DP方案治疗,观察组在此基础上加用康艾注射液、贝伐单抗治疗。比较2组患者治疗效果,治疗前后血清肿瘤标志物、CTC、血管内皮生长因子(VEGF)、免疫功能指标变化,观察2组不良反应发生率。结果治疗后观察组疾病控制率明显高于对照组(79.25%vs.60.47%,χ~2/P=4.053/0.044);与治疗前比较,治疗后2组患者血清CD4~+、CD4~+/CD8~+明显升高,而CA21-1、CEA、CA50、CTC、VEGF、CD8~+水平均明显降低,且观察组改善程度较对照组更为显著(t/P=6.124/0.000,11.317/0.000,14.091/0.000,8.376/0.000,6.866/0.000,18.497/0.000,9.953/0.000,5.439/0.000)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论康艾注射液联合贝伐单抗治疗晚期NSCLC,可以更好地增强机体免疫力,降低血清CA21-1水平及CTC细胞数量,减少新生血管的形成,抑制肿瘤的增殖和转移,效果显著。

【Abstract】 Objective To observe the effect of Kangai injection combined with bevacizumab treatment on circulating tumor cells and cytokeratin 19 fragment(CA21-1) levels in patients with non-small cell lung cancer(NSCLC).Methods A retrospective analysis of the clinical data of 96 patients with NSCLC admitted to the Third Department of Pulmonary Tumor, Hebei Chest Hospital from June 2018 to June 2020,According to different treatment methods, they were divided into two groups: control group(n=43) and observation group(n=53).The control group was treated with DP regimen, and the observation group was treated with Kangai injection and bevacizumab on this basis. The treatment effects, immune function indexes, serum tumor markers, vascular endothelial growth factor(VEGF) and circulating tumor cells(CTC) changes were compared between the two groups before and after treatment, and the incidence of adverse reactions in the two groups was observed.Results The disease control rate of the observation group was significantly higher than that of the control group(79.25% vs.60.47%,χ~2/P=4.053/0.044);Compared with before treatment, after treatment, serum CD4~+, CD4~+/CD8~+ in the two groups were significantly increased, while, CA21-1,CEA, CA50, CTC,VEGF, CD8~+ levels were significantly reduced.and the degree of improvement in the observation group was more significant(t/P=6.124/0.000,11.317/0.000,14.091/0.000,8.376/0.000,6.866/0.000,18.497/0.000,9.953/0.000,5.439/0.000). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05). Conclusion Kangai injection combined with bevacizumab in the treatment of NSCLC can better enhance the body immunity, reduce the level of serum CA21-1 and the number of CTC cells, reduce the formation of new blood vessels, inhibit tumor metastasis and proliferation, with significant effect and certain clinical application value.
