【摘要】 患儿,女,7岁,因"全身红斑鳞屑1年,加重2月"来诊,西医诊断为寻常型银屑病,中医辨证为营卫失和,肌肤失养,少阳兼表证。予柴胡桂枝汤联合中成药治疗5个月后银屑病皮损面积和严重性指数(PASI)改善率为99.27%,且未发现毒副作用,停药后随访半年未复发。回顾文献发现柴胡类制剂对细胞炎症因子水平及表皮的增殖分化有调节作用,但柴胡桂枝汤治疗银屑病的分子生物学机制还需要进一步研究。中西医结合治疗过程中需要随证加减药物,调整剂量,并监测肝、肾功能等,使个体化治疗达到最佳疗效。更多还原

【Abstract】 A 7-year-old girl presented erythema and papules covered by white scales all over the body for 1 year,aggravated in the last two months, diagnosed as psoriasis vulgaris. According to traditional Chinese medicine, the syndrome differentiation was disharmony between Ying and Wei, skin dystrophy and Shaoyang syndrome. The improvement rate of psoriatic lesion area and severity index(PASI) was 99.27% after 5 months of treatment with Chaihu Guizhi decoction combined with Chinese patent medicine, and no toxic and side effects were found. Follow-up lasted half a year without recurrence. Chaihu Guizhi decoction had a regulatory effect on the levels of inflammatory cytokines and epidermal proliferation and differentiation, and the molecular biological mechanism of it in the treatment of psoriasis needs further study. In the process of integrated traditional Chinese and western medicine treatment, drugs should be added or reduced along with the syndrome changes, dosage should be adjusted, and liver and kidney function should be monitored to achieve the best curative effect.更多还原
