【摘要】 目的探讨甲状腺乳头状癌患者术中应用可吸收止血纱的临床效果及对引流液甲状旁腺激素(dPTH)的影响。方法对安徽理工大学第一附属医院2019年12月—2020年1月治疗的单侧甲状腺乳头状癌患者的临床资料进行回顾性分析,共有36例患者,男性6例,女性30例。根据患者术中是否应用可吸收止血纱分为观察组20例和对照组16例。比较两组患者手术时间、术中出血量、肿瘤直径、淋巴结清扫数、术后并发症、住院费用、住院天数、术后住院天数、留置引流管天数及术后24h dPTH间的差异。结果观察组住院天数、手术时间、术中出血量、肿瘤直径、淋巴结清扫数、术后并发症和住院费用分别为(8.95±2.85)d、(117.05±25.91)min、(13.00±6.37)ml、(0.57±0.24)cm、(4.35±1.18)个、1例和(12955.73±2223.02)元,对照组分别为(10.00±2.22)d、(121.13±36.97)min、(15.63±11.24)ml、(0.64±0.25)cm、(4.37±1.26)个、2例和(14788.71±2786.03)元,两组相比差异无统计学意义(P>0.05)。观察组留置引流管天数、术后24h引流液量和术后住院天数分别为(2.90±0.58)d、(50.10±15.67)ml和(3.95±1.15)d,对照组分别为(4.53±1.15)d、(88.63±20.05)ml和(5.19±1.42)d,两组相比差异有统计学意义(P <0.05)。观察组术后24h dPTH为(2210.72±147.11)pg/ml,而对照组为(2190.60±159.93)pg/ml,两组相比差异无统计学意义(P>0.05)。结论甲状腺乳头状癌术中应用可吸收止血纱,可缩短留置引流管时间,缩短术后住院时间;但对术后引流液PTH无影响。更多还原

【Abstract】 Objective To investigate the clinical effect of absorbable hemostatic yarn and its effect on the parathyroid hormone of drainage fluid(dPTH)in patients with papillary thyroid carcinoma.Methods We collected the clinical data of patients with unilateral papillary thyroid carcinoma treated in the First Affiliated Hospital of Anhui University of Science and Technology from December 2019 to January 2020 for retrospective analysis.There were a total of 36 patients,including 6 males and 30 females.The patients were divided into an observation group(n =20)and a control group(n =16)according to whether the absorbable hemostatic yarn was applied during the operation.The differences in operation duration,intraoperative blood loss volume,tumor diameter,number of lymph node dissection,postoperative complications,hospitalization expenses,inpatient days,postoperative inpatient days,days of indwelling drainage tube and dPTH at 24 hours after operation were compared between two groups.Results The inpatient days,operation duration,intraoperative blood loss volume,tumor diameter,number of lymph node dissection,postoperative complications and hospitalization expenses of patients in the observation group were(8.95±2.85)d,(117.05±25.91)min,(13.00±6.37)ml,(0.57±0.24)cm,(4.35±1.18),1 case,and(12,955.73±2223.02)yuan,respectively.Those in the control group were(10.00±2.22)d,(121.13±36.97)min,(15.63±11.24)ml,(0.64±0.25)cm,(4.37±1.26),2 cases,and(14788.71±2786.03)yuan,respectively.There was no significant difference between two groups in these indexes(P >0.05).The days of indwelling drainage tube,total volume of drainage fluid at 24 hours after surgery and inpatient days after surgery were(2.90±0.58)d,(50.10±15.67)ml,and(3.95±1.15)d,respectively in the observation group,whereas those in the control group were(4.53±1.15)d,(88.63±20.05)ml and(5.19±1.42)d.The differences in these aspects between two groups were statistically significant(P <0.05).The dPTH at 24 hours after surgery in the observation group was(2210.72±147.11)pg/ml,while that of the control group was(2190.60±159.93)pg/ml,which indicated no significant difference between two groups(P >0.05).Conclusion The application of absorbable hemostatic yarn in the surgery of papillary thyroid carcinoma can shorten the indwelling time of the drainage tube and shorten postoperative hospitalization duration.But it has no effect on the PTH of postoperative drainage fluid.更多还原
