血清25-羟维生素D、中性粒细胞/淋巴细胞比值与乙肝肝硬化的相关性分析

【摘要】 目的探究血清25-羟维生素D、中性粒细胞/淋巴细胞比值(NLR)与乙肝肝硬化(HBV)的相关性。方法选取2018年9月至2020年6月期间就诊于锦州市中心医院患者160例,其中健康对照组为70例,乙肝肝硬化患者90例为实验组,并根据Child-Pugh评分、终末期肝病模型(MELD)评分对实验组进行分组。收集患者一般资料,所有入组患者进行血常规、肝肾功、凝血四项、25-羟维生素D检测,计算NLR,分析乙肝肝硬化患者血清中25-羟维生素D、NLR水平,并分析其与乙肝肝硬化的相关性及两个指标之间的相关性。结果乙肝肝硬化组25-羟维生素D低于健康对照组,而NLR高于健康对照组(P<0.05)。根据Child-Pugh评分、终末期肝病模型(MELD)评分将实验组分别分成三组,Child-Pugh评分、MELD评分中的25-羟维生素D水平随着评分升高逐渐降低、NLR随着评分升高而升高,上述组间差异均有统计学意义(P<0.05)。运用Spearman相关性分析发现,Child-Pugh评分、MELD评分与25-羟维生素D成负相关(r=-0.679、r=-0.624,均P<0.01),与NLR成正相关(r=0.566、r=0.535,均P<0.01),25-羟维生素D与NLR呈负相关(r=-0.525, P<0.01)。结论 25-羟维生素与肝硬化严重程度呈负相关;NLR与肝硬化的严重程度呈正相关,25羟维生素D水平影响NLR。

【Abstract】 【Objective】 To explore the serum levels of 25-hydroxyvitamin D and neutrophil-lymphocyte ratio(NLR) and their correlation in patients with hepatitis B liver cirrhosis. 【Methods】 A total of 160 patients visited Jinzhou Central Hospital between September 2018 and June 2020 were selected, of whom 70 were included in the healthy control group, and 90 hepatitis B cirrhosis patients were included in the experimental group, and were assessed according to the Child-Pugh score and MELD score. The experimental group were divided into subgroups. The general data of the patients were collected. All the enrolled patients were tested for blood routine, liver and kidney function, blood coagulation, 25-hydroxyvitamin D, the NLR ratio was calculated, the 25-hydroxyvitamin D and NLR levels in the serum of patients with hepatitis B cirrhosis were analyzed, and its correlation with hepatitis B cirrhosis and the correlation between the two indicators were analyzed. 【Results】 The 25-hydroxyvitamin D was lower, while the NLR was higher in the hepatitis B cirrhosis group than that in the healthy control group(P<0.05). According to the Child-Pugh score and the Model for End-stage Liver Disease(MELD) score, the experimental group was divided into 3 groups, respectively.The 25-hydroxyvitamin D level in the Child-Pugh score and the MELD score gradually decreased as the score increased, and the NLR increased with the increase of the score, the differences between the above groups were statistically significant(P<0.05). Using Spearman correlation analysis, it was found that Child-Pugh score and MELD score were negatively correlated with 25-hydroxyvitamin D(r=-0.679, r=-0.624, both P<0.01), and positively correlated with NLR(r=0.566, r=0.535, both P<0.01), 25-hydroxyvitamin D was negatively correlated with NLR(r=-0.525, P<0.01). 【Conclusion】 25-hydroxyvitamin is negatively correlated with the severity of liver cirrhosis; NLR ratio is positively correlated with the severity of liver cirrhosis, 25-hydroxyvitamin D level affects the NLR ratio.
