三血管及三血管气管切面在超声产前诊断胎儿心脏大血管畸形中的应用研究
Application value of the Three vessel view and the Trachea section of three vessels in the prenatal ultrasound diagnosis of fetal cardiomacrovascular malformations
【摘要】 目的 分析三血管切面(Three vessel view,3VV)及三血管气管切面(Trachea section of three vessels,3VT)在超声产前诊断胎儿心脏大血管畸形中的应用价值。方法收集本院2018年1月~2019年1月在本院行产前筛查的孕妇3142例,均进行3VV及3VT检查。观察产妇超声检查结果,了解胎儿心脏畸形况并妥善保留相关检查数据,将检查结果与产后(或引产)随访结果进行对比,分析3VV及3VT检查在筛查胎儿心脏大血管畸形中诊断准确度。结果 3142例患者中在后期产妇均未失访,经引产后病理检查或产后检查确诊有71例(2.26%)心脏大血管畸形:血管位置异常18例,血管管径大小异常37例,血管数量异常16例。产前超声检查中有65例(2.07%)疑似为心脏大血管畸形,血管位置异常17例,漏诊1例,检出率为94.44%;血管管径大小异常34例,漏诊3例,检出率为91.89%,血管数量异常14例,漏诊2例,检出率为87.5%。与随访结果比较并无差异,(P>0.05)。71例患者中有15例合并存在其他畸形,其中脑畸形2例,唇腭裂4例,肢体畸形4例,脑积水1例,脊柱裂1例,胸腔积液2例,内脏反位1例。结论 3VV及3VT切面在超声产前诊断胎儿心脏大血管畸形中的应用价值高,且操作简便,可作为产前基本筛查切面之一。
【Abstract】 Objective:To analyze the application value of the Three vessel view (3VV) and the Trachea section of three vessels (3VT) in the prenatal ultrasound diagnosis of fetal cardio-macrovascular malformations.Methods:3142 pregnant women who had prenatal screening in this hospital from January 2018 to January 2019 were collected.All patients underwent 3VV and 3VT examinations.The results of maternal ultrasound examinations are observed to understand the fetal heart malformations and properly retain the relevant examination data.The examination results were compared with the postpartum (or labor induction) follow-up results,and the diagnostic accuracy of 3VV and 3VT examinations in the screening of fetal cardio-macrovascular malformations was analyzed.Results Among the 3142 patients,no women were lost to follow-up at the later stage.There were 71 cases (2.26%) with cardio-macrovascular malformations diagnosed by pathological examinations after induction of labor or postpartum examinations:18 cases with abnormal position of blood vessel,37 cases with abnormal diameter of blood vessel,and 16 cases with abnormal number of blood vessel.In the prenatal ultrasound examination,65 cases (2.07%) were suspected to have cardio-macrovascular malformations,17 cases had abnormal position of blood vessel and1 case was missed diagnosis,the detection rate was 94.44%;37 cases had abnormal diameter of blood vessel,3 cases were missed diagnosis,and detection rate was 91.89%;14 cases had abnormal number of blood vessel,2 cases were missed diagnosis,and detection rate was 87.5%.There was no difference between it and the follow-up results(P>0.05).Among 71 patients,15 cases had other malformations,including 2 cases with brain malformations,4 cases with cleft lip and palate,4 cases with limb malformations,1 case with hydrocephalus,1 case with spina bifida,2 cases with pleural effusion,and 1 case with situs inversus.Conclusion 3VV and 3VT have high application value in prenatal ultrasound diagnosis of fetal cardio-macrovascular malformations,and they are easy to operate.They can be used as one of the basic section of prenatal screening.
