【摘要】 目的系统评价治疗前预后营养指数(PNI)与乳腺癌患者预后的相关性。方法系统检索PubMed、Cochrane library、Embase、Web of science、Wangfang data、VIP、CNKI等电子数据库,纳入相关文献。两名研究人员独立筛选文献、提取有效资料并行质量评价。对于PNI及其他临床参数和总生存期(OS)、无病生存期(DFS)的相关性研究,以及PNI和乳腺癌患者临床病理特征的相关性研究,进行meta分析。结果最终纳入4篇文献,共1 165例患者。与治疗前PNI低组相比,PNI高组的OS、DFS明显延长,差异均有统计学意义(风险比=0.42、0.64,P=0.000 2、0.02)。治疗前低PNI与年龄、肿瘤分期、组织学分型、是否复发、是否行新辅助化疗均不存在明显相关性(P>0.05)。结论治疗前低PNI与乳腺癌患者的不良预后明显相关,但与年龄、肿瘤分期、组织学分型等临床参数无明显相关性。而治疗前高PNI预示着OS、DFS更好。更多还原

【Abstract】 Objective To systematically review the prognostic value of the pre-treatment prognostic nutritional index(PNI)for patients with breast cancer.Methods The PubMed,Cochrane library,Embase,Web of science,Wangfang data,VIP,CNKI and other electronic databases were searched systematically,and including relevant literature.Two researchers independently screened the literature,extracted valid data and evaluated the quality.For PNI and other clinical parameters,overall survival(OS),disease-free survival(DFS)correlation research,as well as PNI and breast cancer patients clinical pathological characteristics of the correlation research,conduct a meta-analysis.Results 4 articles were included,with a total of 1 165 patients.Compared with the low pre-treatment PNI group,OS and DFS of the high PNI group were significantly prolonged,and the differences were statistically significant(hazard ratio=0.42,0.64,P=0.000 2,0.02).There was no significant correlation between low pre-treatment PNI and age,tumor stage,histological type,recurrence,and neoadjuvant chemotherapy(P>0.05).Conclusion A low pre-treatment PNI was significantly associated with poor prognosis in patients with breast cancer,and it has no obvious correlation with clinical parameters such as age,tumor stage,histological type,etc.A high pre-treatment PNI is indicated better OS and DFS in patients.更多还原
