11C-胆碱PET/CT对颅内占位性病变良恶性鉴别诊断的价值

【摘要】 目的研究11C-胆碱(11C-CHO)正电子发射断层显像/X线计算机体层显像(PET/CT)对颅内占位性病变良恶性鉴别诊断的价值。方法选用怀疑有颅内占位性病变的患者29例,入选患者在3 d内同期行18F-氟代脱氧葡萄糖(18F-FDG)和(11C-CHO)PET/CT显像,采用视觉分析及半定量分析方法,用病灶/对侧正常脑组织比值的最大标准化摄取值(SUVmax)>1作为阳性诊断标准,测出病灶及对侧脑组织的SUVmax。结果 (1)11C-CHO和18F-FDG的精确度、灵敏度、特异度分别为91.5%、100%、50%和53.2%、50%、61.5%;(2)18F-FDG和11C-CHO所得的SUVmax受试者工作特征曲线下的面积分别为0.93、0.74;(3)11C-CHO假阳性病例4例。结论11C-CHO PET/CT对颅内占位性病变良恶性鉴别诊断具有较高的临床应用价值,但仍存在一定的假阳性。

【Abstract】 Objective To study differential diagnosis of the value of 11C-choline PET/CT in benign and malignant intracranial space-occupying lesions, so as to improve the accuracy of PET in diagnosis of intracranial space-occupying lesions. Methods Twenty-nine patients with suspected intracranial space occupying lesions were selected. 18F-FDG and 11C-CHO PET/CT imaging agent were performed simultaneously within 3 days. Visual analysis and semi-quantitative methods were used. The SUVmax ratio of the lesion to the contralateral normal brain tissue>1 was used as the positive diagnostic criterion, and the SUVmax of the lesion was measured. Results(1) The sensitivity, specificity and accuracy of 11C-CHO and 18F-FDG were 91.5%, 100%, 50% and 53.2%, 50%, 61.5% respectively.(2) The area under the curve of SUVmax 18F-FDG and 11C-CHO were 0.74 and 0.93 respectively.(3) There were 4 false positive cases of 11C-CHO. Conclusion 11C-CHO PET/CT has high clinical value in the differential diagnosis of benign and malignant intracranial space occupying lesions, but there are still some false positives.
