【摘要】 目的探讨生肌橡皮膏治疗气虚血瘀型下肢静脉性溃疡的效果及对创面愈合的影响。方法选取2019年1月至2020年9月于石家庄市中医院接受治疗的115例气虚血瘀型下肢静脉性溃疡患者为研究对象,按照随机数字表法分为观察组(n=58)和对照组(n=57)。比较两组患者治疗前后的临床症状和体征积分、中医证候疗效、细菌培养阳性率、炎性因子[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、肿瘤坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)]水平与生活质量[下肢静脉性溃疡患者生命质量问卷(VLU-QoL)]评分及治疗过程中的不良反应情况。结果治疗后,观察组患者的创面面积积分高于对照组患者,溃疡表面色泽、创面渗液量和新生肉芽组织积分低于对照组患者;中医证候疗效有效率、日常生活影响、主观感受和局部症状评分高于对照组患者,细菌培养阳性率、IL-6、IL-1β、TNF-α、hs-CRP水平低于对照组患者,差异有统计学意义(P<0.05)。治疗过程中,两组均未出现不良反应。结论生肌橡皮膏治疗气虚血瘀型下肢静脉性溃疡可有利于患者炎性因子的控制及降低细菌感染率,有效改善其临床症状和体征,提高临床疗效及生活质量,值得推广。更多还原

【Abstract】 Objective To explore the effects of Shengji Xiangpi ointment in the treatment of venous ulcers of lower extremities(qi deficiency and blood stasis type) and its influence on wound healing. Method A total of 115 patients with lower extremities venous ulcer of qi deficiency and blood stasis type treated in Shijiazhuang Hospital of Traditional Chinese Medicine from January 2019 to September 2020 were randomly divided into observation group(n=58) and control group(n=57), according to random number table method. The scores of clinical symptoms and signs, curative effect of TCM symptoms, positive rate of bacterial culture, inflammatory factors [interleukin-6(IL-6), interleukin-1β(IL-1β), tumor necrosis factor α(TNF-α), high sensitivity C-reactive protein(hs-CRP)] and quality of life [venous leg ulcer quality of life questionnaire(VLU-QoL)] before and after treatment and incidence rate of adverse reactions during treatment were recorded among the patients. Result After treatment, the wound area score in the observation group was higher than that in the control group, and the ulcer surface color, wound exudation and new granulation tissue score in the observation group were lower than those in the control group. The effective rate of TCM symptoms, the impact of daily life, subjective feelings and local symptom scores were higher than those in the control group; positive rate of bacterial culture, the level of IL-6, IL-1β, TNF-α, hs-CRP was lower than that in the control group; the difference was statistically significant(P<0.05). During the treatment, there were no adverse reactions in both groups. Conclusion Shengji Xiangpi ointment for venous ulcers of lower extremities(qi deficiency and blood stasis type) can help patients control inflammatory factors and reduce bacterial infection rate, effectively improve the clinical symptoms and signs, and enhance clinical efficacy and quality of life, therefore it is worthy of promotion.更多还原
