开窗与“烟囱”EVAR技术治疗腹主动脉瘤的中远期疗效研究进展

【摘要】 腹主动脉瘤(AAA)是较常见的动脉瘤疾病类型,病情较急,需要及时治疗。随着医疗技术的发展,越来越多的治疗方法被采用,腔内修复术是目前治疗AAA的常用方法,并且在临床中应用广泛,具有治愈率较高、并发症较少的优势。但需要注意的是,部分患者的情况并不满足既往腔内修复术的治疗条件,如破口较大、破口位于主动脉弓小弯侧、锚定区血管条件差等原因致锚定区不足的情况均无法使用普通的腔内修复术。基于此,“烟囱”和开窗支架被应用于AAA的腔内修复治疗。虽然开窗和“烟囱”技术的治疗效果较佳,但仍存在不足之处。本文就腔内修复术的进展、AAA开窗和“烟囱”的治疗效果、开窗和“烟囱”预后的并发症及处理等相关问题进行系统综述,为进一步的研究与应用提供参考。

【Abstract】 Abdominal aortic aneurysm(AAA) is a more common type of aneurysm disease, which is more urgent and requires timely treatment. With the continuous development of medical technology, more and more treatments have been adopted. Currently, the most common treatment for AAA is endovascular repair, which is widely used clinically with higher cure rate and lower complications. But it should be noted that the conditions of some patients do not meet the treatment criteria of previous endovascular repairs, such as large ruptures, ruptures located on the side of the minor curvature of the aortic arch, poor vascular conditions in the anchoring area. If the anchoring area is insufficient,conventional endovascular repair cannot be used. Based on these factors, chimneys and fenestration brackets have been applied to the endovascular repair treatment of AAA. Although the treatment effect of fenestration and chimney technology is better, there are still shortcomings. This article systematically reviews the progress of endovascular repair,the therapeutic effect of AAA with fenestration and chimney technology, the prognosis and management of complications for fenestration and chimney technology, and provides a reference for further research and application.
