经桡动脉冠状动脉造影患者术后早期双联抗血小板治疗对桡动脉管腔丢失率的影响
Influence of early postoperative dual antiplatelet therapy on radial artery lumen loss in patients with transradial coronary angiography
【摘要】 目的:采用前瞻性研究方法比较不同抗血小板方案对经桡动脉冠状动脉造影患者桡动脉的管腔丢失率的影响。方法:入选2019年12月至2021年12月期间,于我院行经桡动脉冠状动脉造影的患者共200例。根据术后抗血小板方案将患者分为单抗组(n=112)和双抗组(n=88)。使用超声采集术前桡动脉基线数据及术后1个月桡动脉随访数据。通过桡动脉内径变化判断术后短期抗血小板方案对桡动脉管腔丢失率的影响。结果:两组患者在基线人口学资料及手术相关资料方面,差异无统计学意义,术后双抗组桡动脉内径丢失率明显低于单抗组(42.8%vs. 23.4%,P<0.001),其桡动脉狭窄发生率也较单抗组明显减低(36.6%vs. 11.4%,P<0.001)。结论:经桡动脉冠状动脉造影术后短期双联抗血小板治疗可减少桡动脉管腔丢失率,有助于桡动脉术后的损伤修复。
【Abstract】 Objective: A prospective observational cohort study was used to compare the protective effects of different antiplatelet regiments on the radial artery in patients with transradial coronary angiography. Methods: A total of 200 patients who underwent transradial coronary angiography in our hospital from December 2019 to December 2021 were enrolled. According to postoperative antiplatelet regimen, patients were divided into monoclonal antibody group(n=112) and dual antiplatelet group(n=88). Preoperative baseline data and postoperative follow-up data of the radial artery were collected by ultrasound. The protective effect of short-term postoperative antiplatelet regimen on radial artery injury was evaluated by radial artery diameter change. Results: There were no significant differences in baseline demographic data and surgical data between the two groups. The postoperative radial artery diameter loss rate in the dual-antibody group was significantly lower than that in the single-antibody group(42.8% vs. 23.4%, P<0.001), and the incidence of radial artery stenosis was significantly lower than that in the single-antibody group(36.6% vs. 11.4%, P<0.001). Conclusions: Short-term dual antiplatelet therapy after transradial coronary angiography can reduce the lumen loss rate of the radial artery and help repair the postoperative injury of the radial artery.
