食管癌围手术期上消化道出血的防范与处理
Prevention and treatment of upper gastrointestinal bleeding during perioperative period of esophageal carcinoma
【摘要】 目的:探讨食管癌患者围手术期上消化道出血的高危因素、防范与处理。方法:回顾性分析2016年至2020年,行食管癌根治术的826例患者的临床资料,共有25例患者发生围手术期上消化道出血,对上消化道出血的原因及治疗方法进行分析。结果:25例患者中,10例行急诊胃镜检查,吻合口出血4例,残胃出血5例,十二指肠溃疡1例,开胸止血2例,余保守治疗,均顺利出院。结论:术中仔细止血,术后密切观察是防治食管癌围手术期上消化道出血的关键,一旦发现出血及时采取措施,尽早急诊胃镜检查,必要时开胸止血治疗。
【Abstract】 Objective: To explore the high risk factors, prevention and treatment of upper gastrointestinal bleeding in patients with esophageal carcinoma during perioperative period. Methods: The clinical date of 826 patients who were underwent esophagectomy from 2016 to 2020 were analyzed retrospectively. 25 patients had upper gastrointestinal bleeding during the perioperative period. The causes and treatment of upper gastrointestinal hemorrhage were analyzed. Results: Among 25 patients, emergency gastroscopy in 10 patients, anastomotic bleeding in 4 patients, gastric stump bleeding in 5 patients, duodenal ulcer in 1 patients. 2 patients underwent thoracotomy and hemostasis. Other patients were treated conservatively and discharged smoothly. Conclusions: Careful hemostasis during operation and close observation after operation are the key to prevent and treat the bleeding of upper digestive tract in the perioperative period of esophageal carcinoma. If bleeding is found, we should take measures in time, emergency gastroscopy as soon as possible, and thoracotomy for hemostasis if necessary.
