【摘要】 目的对医院住院患者使用伏立康唑的合理性进行评价,为临床合理用药提供依据。方法根据伏立康唑的药品说明书、相关指南、文献及该院临床用药实际情况,制定伏立康唑用药评价标准。调取该院2019年1月—2020年12月使用伏立康唑的出院患者病历,对其用药指征、用法用量、给药疗程、不良反应、血药浓度监测情况等进行回顾性分析。结果共抽取310例,其中重症医学科及血液内科占69.04%。经分析,合理用药183例,占59.03%。不合理用药表现为:用法用量不合理(47.66%),无用药指征(19.57%),特殊人群未监测血药浓度(19.15%)及选用的药品不合理(13.62%)。结论医院普遍存在伏立康唑不合理使用现象,需进一步加强伏立康唑合理使用的宣教及点评,以确保用药的有效性和安全性。更多还原

【Abstract】 Objective To evaluate the rationality of voriconazole for clinical use among inpatients in our hospital so as to provide reference for clinical drug use.Methods The evaluation criteria for medication of voriconazole were formulated according to the drug instructions, related guidelines, literature and the actual situation of clinical medication in the hospital. The medical records of discharged patients treated with voriconazole in the hospital from January 2019 to December 2020 were retrieved and retrospectively analyzed for the medication indications, usage and dosage, duration of administration, adverse effects, and blood concentration monitoring.Results A total of 310 cases were extracted, of which 69.04% were from ICU and Hematology Department. After analysis, 183 cases were identified as rational medication, accounting for 59.03%. Irrational drug use was found to be unreasonable use and dosage(47.66%), no indications for medication(19.57%), unmonitored blood drug concentration for special population(19.15%), and unreasonable selection of drugs(13.62%).Conclusion Irrational use of voriconazole is common in the hospital. It is necessary to strengthen the education and review on the rational use of voriconazole to ensure the effectiveness and safety of medication.更多还原
